Appendectomy For Appendicitis In Child - Pediatric Appendix
Surgery / Sep 1st, 2020 11:17 am     A+ | a-

This video demonstrate a cut or incision is made in the lower right-hand side of the belly. The surgeon finds the appendix and removes it. If the appendix has burst, a small tube or shunt may be placed to drain out pus and other fluids from the belly.

Laparoscopic appendectomy in the pediatric population is more than just a "smaller version" of adult surgery. Children have unique anatomical challenges, such as a smaller intra-abdominal workspace and a thinner abdominal wall. At World Laparoscopy Hospital, the approach to a pediatric appendix focuses on minimizing trauma, maximizing aesthetics, and ensuring a rapid return to normal childhood activities.

The Pediatric Challenge: Why Laparoscopy?

In children, the diagnosis of appendicitis can be tricky because they often present with vague symptoms. Laparoscopy serves a dual purpose: it confirms the diagnosis visually and allows for immediate treatment.

  • Anatomic Advantage: Children have a relatively shorter distance between the umbilicus and the appendix, making laparoscopic access highly efficient.

  • Reduced Trauma: Unlike the traditional "McBurney’s" open incision, which can be painful and lead to muscle weakness, laparoscopic ports are tiny and heal with almost no visible scarring.

Surgical Techniques for Children

1. The Three-Port Technique

This is the standard approach at WLH.

  • A 5 mm or 10 mm umbilical camera port.

  • Two 3 mm or 5 mm working ports in the lower abdomen.

  • Benefits: The use of 3 mm "microlaparoscopic" instruments is ideal for smaller abdominal cavities, reducing post-operative pain to a minimum.

2. Single Incision Laparoscopic Surgery (SILS)

For the ultimate cosmetic result, the appendix is removed entirely through a single incision in the umbilicus (belly button). Once healed, the scar is hidden within the natural folds of the navel, making the surgery "scarless."

3. Management of the Mesoappendix and Stump

In children, the mesoappendix (the fatty tissue containing the blood supply) is usually thinner than in adults.

  • Mishra’s Knot: As discussed previously, this intracorporeal knot is excellent for children because it avoids the need for bulky, expensive staplers that might be too large for a small cecum.

  • Bipolar Cautery: Often used to safely coagulate the small vessels in the mesoappendix.

Safety and Specialized Care at WLH

Pediatric cases at World Laparoscopy Hospital follow strict safety protocols:

  • Low-Pressure Pneumoperitoneum: The carbon dioxide pressure used to inflate the abdomen is kept lower ($8\text{--}10 \text{ mmHg}$ compared to $12\text{--}15 \text{ mmHg}$ in adults) to prevent unnecessary pressure on a child’s diaphragm and heart.

  • Thermoregulation: Children lose heat quickly in the OR. WLH utilizes warmed CO2 and thermal blankets to keep the patient's core temperature stable.

  • Nerve Blocks: Surgeons often perform a TAP (Transversus Abdominis Plane) block under direct vision before finishing, ensuring the child wakes up virtually pain-free.

Recovery: Back to Being a Kid

The recovery timeline for a pediatric laparoscopic appendectomy is remarkably fast:

  • Hospital Stay: Usually 12 to 24 hours.

  • Diet: Clear liquids are started within hours, progressing to a regular diet by the next morning.

  • Activity: Most children are back to school within 3 to 5 days, though contact sports are usually restricted for 2 weeks.

Conclusion

Laparoscopic appendectomy is the gold standard for pediatric appendicitis. By combining advanced technology with the specialized "Mishra’s Knot" and microlaparoscopic tools, World Laparoscopy Hospital ensures that the surgical experience is as gentle as possible for the youngest patients.
