Laparoscopic Dermoid Ovarian Cystectomy - Ovarian Teratoma

This video demonstrate laparoscopic surgery for ovarian dermoid. A dermoid cyst is a saclike growth that is present at birth. It contains structures such as hair, fluid, teeth, or skin glands that can be found on or in the skin. Dermoid cysts grow slowly and are not tender unless ruptured. They usually occur on the face, inside the skull, on the lower back, and in the ovaries.

Laparoscopic dermoid ovarian cystectomy is a minimally invasive surgical procedure performed to remove dermoid cysts (also called mature cystic teratomas) from the ovary while preserving normal ovarian tissue. Dermoid cysts are among the most common benign ovarian tumors, particularly in women of reproductive age, and may account for a significant proportion of ovarian masses. Although usually non-cancerous, surgical removal is often recommended to relieve symptoms and prevent complications such as torsion, rupture, or rare malignant transformation.

A dermoid cyst is unique because it originates from germ cells and can contain tissues like hair, fat, skin, or teeth. Many patients remain asymptomatic, and cysts are often detected incidentally during ultrasound examinations. When symptoms occur, they may include pelvic pain, abdominal swelling, menstrual irregularities, or pressure symptoms. Larger cysts may increase the risk of ovarian torsion or rupture, which are surgical emergencies.

Laparoscopic cystectomy has become the preferred treatment for most benign ovarian dermoid cysts. In this procedure, small abdominal incisions are made to insert a camera and specialized instruments. The cyst is carefully dissected from the ovary and removed, usually with preservation of healthy ovarian tissue, which is especially important for fertility. In selected cases, such as suspicion of malignancy or severe ovarian damage, partial or total removal of the ovary may be required.

The laparoscopic approach offers several advantages over traditional open surgery. These include smaller incisions, reduced postoperative pain, shorter hospital stay, faster recovery, and better cosmetic outcomes. Many patients are discharged within 1–2 days and return to normal activities relatively quickly. Studies have shown that laparoscopic removal of dermoid cysts is safe, with low rates of major complications when performed by experienced surgeons.

However, certain intraoperative challenges may occur, such as rupture of the cyst and spillage of its contents into the abdominal cavity. Although spillage is relatively common in larger cysts, careful surgical technique and thorough peritoneal wash can minimize complications like chemical peritonitis, which is rare. Other possible complications include port-site bleeding, infection, or temporary bowel dysfunction, but these are generally manageable.

Overall, laparoscopic dermoid ovarian cystectomy is considered a safe and effective fertility-preserving procedure for treating ovarian teratomas. Early diagnosis, proper patient selection, and skilled laparoscopic surgical technique play key roles in achieving excellent clinical outcomes.
