This video demonstrate Laparoscopic Appendicectomy & Total Laparoscopic Hysterectomy in Same patient performed by Dr R K Mishra at World Laparoscopy Hospital. With advancements in minimal access surgery, combined laparoscopic procedures are now being performed for treating coexisting abdominal pathologies at the same surgery.

Minimally invasive surgery has revolutionized modern surgical practice by allowing complex procedures to be performed safely through small incisions. Two common laparoscopic procedures—laparoscopic appendicectomy and total laparoscopic hysterectomy (TLH)—are often performed independently. However, in selected patients, performing both procedures in the same operative sitting is feasible, safe, and clinically beneficial. This combined approach reduces anesthesia exposure, shortens hospital stay, and improves overall patient recovery.

Laparoscopic appendicectomy is the gold standard treatment for acute appendicitis, offering reduced postoperative pain, shorter hospitalization, and faster recovery compared to open surgery.

Total laparoscopic hysterectomy is widely accepted for managing various gynecological conditions such as fibroids, adenomyosis, abnormal uterine bleeding, and early-stage malignancies. Clinical trials have shown that laparoscopic hysterectomy provides outcomes equivalent to open surgery in survival and disease control, supporting its routine use.

With advancements in laparoscopic techniques, surgeons are increasingly performing combined procedures to address multiple pathologies during a single anesthesia session.

Rationale for Performing Both Procedures Together

Performing laparoscopic appendicectomy and TLH together may be indicated in:

  • Acute appendicitis in a patient planned for hysterectomy

  • Chronic right lower quadrant pain with gynecological pathology

  • Incidental appendectomy during planned hysterectomy

  • Suspected appendix pathology detected intraoperatively

  • Preventive appendectomy in selected patients

Studies evaluating incidental appendectomy during TLH show no increase in complications, similar blood loss, and even shorter hospital stay in some cases.

Additionally, research on combined laparoscopic gynecological and digestive procedures demonstrates comparable recovery time and hospital stay compared to single procedures, confirming the benefits of combined surgery.

Safety and Feasibility

Modern evidence supports the safety of performing combined laparoscopic procedures. Large surgical series combining TLH with other abdominal procedures (such as hernia repair) have shown low complication rates, no mortality, and acceptable morbidity, supporting the concept of multi-procedure laparoscopy.

Similarly, incidental appendectomy during TLH has not been associated with procedure-specific complications and can be routinely offered in selected cases.

These findings highlight that combining laparoscopic procedures is safe when performed by experienced surgeons with proper patient selection.

Surgical Technique Overview

1. Patient Positioning

  • Lithotomy with Trendelenburg position

  • Proper padding and safety checks

2. Port Placement

  • Typically similar to TLH port placement

  • Appendix can usually be accessed through the same ports

3. Sequence of Surgery
Usually depends on pathology and contamination risk:

  • Clean procedure first (TLH)

  • Then contaminated procedure (appendicectomy), if acute appendicitis present

4. Key Steps

  • TLH: Uterine artery ligation → Colpotomy → Specimen removal → Vault closure

  • Appendicectomy: Mesappendix division → Appendix ligation → Specimen retrieval

5. Specimen Retrieval

  • Appendix via endobag

  • Uterus via vaginal route or morcellation if indicated

Advantages of Combined Surgery

1. Single Anesthesia Exposure
Reduces anesthesia-related risks and improves patient convenience.

2. Shorter Total Hospital Stay
Combined surgery avoids two separate admissions.

3. Faster Overall Recovery
Laparoscopy reduces pain, tissue trauma, and postoperative complications.

4. Cost-Effective
Single hospitalization and operative session reduce healthcare costs.

5. Prevention of Future Appendicitis
Incidental appendectomy may prevent future emergency surgery.

Challenges and Considerations

Despite advantages, certain factors must be considered:

  • Longer operative time

  • Risk of contamination if appendicitis is perforated

  • Need for advanced laparoscopic skills

  • Patient comorbidities

  • Presence of severe adhesions

Proper case selection and surgical expertise are crucial for optimal outcomes.

Postoperative Care

Postoperative management includes:

  • Early mobilization

  • Pain management

  • DVT prophylaxis

  • Early oral intake

  • Monitoring for infection or bleeding

Most patients can be discharged within 24–48 hours depending on intraoperative findings.

Future Perspectives

With advancements in robotic surgery, imaging guidance, and enhanced recovery protocols, combined laparoscopic procedures are expected to become more common. The trend is moving toward single-session multi-organ minimally invasive surgery for improved patient-centered care.

Conclusion

Performing laparoscopic appendicectomy and total laparoscopic hysterectomy in the same patient is a safe and effective approach in selected cases. Evidence shows no significant increase in complications and highlights the benefits of reduced hospital stay, faster recovery, and cost-effectiveness. With proper patient selection, surgical planning, and expertise, combined laparoscopic surgery represents the future direction of minimally invasive multi-specialty surgical care.

Very nice. This lady had a great abdominal cavity for teaching and you did a great job explaining things. I am sad that she had conditions that led her to need the surgeries. Even though it would be difficult, could you do a similar surgery on an overweight lady?
