This video demonstrates the Laparoscopic Management of Suprapubic Incisional  Hernia by Dr. R K Mishra at World Laparoscopy Hospital. The suprapubic hernia is the term to describe ventral hernias located less than 4 cm above the pubic arch in the midline. Hernias with an upper margin above the arcuate line encounter technical difficulties, and the differences in repair methods forced us to define them as large suprapubic hernias. Laparoscopic repair of large suprapubic hernias can be considered as the first option in treatment. The low recurrence rates reported in the literature and the lack of recurrence, as observed in our video.

Suprapubic incisional hernias are a challenging subset of abdominal wall hernias, often arising after lower abdominal surgeries such as cesarean sections, hysterectomies, or urological procedures. Their proximity to the pubic bone, bladder, and lower abdominal structures makes traditional open repair technically demanding and prone to complications. Over the past decade, laparoscopic techniques have emerged as a safe, effective, and minimally invasive alternative for the management of these hernias.

At the World Laparoscopy Hospital, pioneering surgeons have refined laparoscopic approaches to suprapubic incisional hernia repair, emphasizing reduced morbidity, faster recovery, and lower recurrence rates. The procedure typically begins with the careful creation of pneumoperitoneum, followed by the placement of strategically positioned ports to provide optimal visualization of the lower abdominal wall. Advanced laparoscopic instruments allow meticulous dissection around critical structures, including the bladder and iliac vessels, minimizing the risk of injury.

One of the key advantages of laparoscopic repair is the placement of a large prosthetic mesh in a tension-free manner. The mesh can be secured using a combination of tackers and sutures, ensuring stable reinforcement while avoiding excessive tissue trauma. The laparoscopic approach also provides excellent magnified visualization, facilitating precise adhesion release and repair of the hernia defect. Patients benefit from smaller incisions, reduced postoperative pain, and significantly faster return to daily activities compared to conventional open surgery.

The World Laparoscopy Hospital has documented excellent outcomes in patients undergoing laparoscopic management of suprapubic incisional hernias. With the integration of preoperative imaging, individualized surgical planning, and state-of-the-art minimally invasive techniques, the hospital consistently achieves low recurrence rates and high patient satisfaction. Moreover, the training programs at the institute enable surgeons worldwide to master these complex procedures under expert guidance, promoting the adoption of advanced laparoscopic skills globally.

In conclusion, laparoscopic management of suprapubic incisional hernia represents a significant advancement in hernia surgery. By combining minimally invasive techniques, meticulous surgical planning, and modern mesh technology, surgeons at the World Laparoscopy Hospital offer patients a safe, effective, and reproducible solution for this challenging condition. This approach not only enhances surgical outcomes but also sets a benchmark in the field of modern hernia repair.
