Laparoscopy is a well-established diagnostic and therapeutic modality for adult gynecologic surgery. We have sought to assess the feasibility of the laparoscopic approach to a particular gynecologic procedure, oophorectomy, in the pediatric population. This patient underwent laparoscopic oophorectomy. The indication for oophorectomy, in this case, was ovarian torsion in association with a benign teratoma, and that occurred prenatally. The patient had a teratoma with significant solid components. Oophorectomy was successfully completed laparoscopically in the patient. Postoperative recovery was prompt, with time to discharge on the same days. From this experience, we conclude that laparoscopic oophorectomy can be successfully accomplished in infants and children. It is technically an easy procedure and has the benefits of excellent visualization of the entire lower abdomen and pelvis including the contralateral ovary, rapid postoperative recovery, and good cosmetic results.

Laparoscopic oophorectomy in very young pediatric patients is a highly delicate and technically demanding procedure that requires advanced surgical skill, specialized instruments, and careful perioperative management. Performing this surgery in a 3-year-old child represents a significant achievement in minimally invasive pediatric surgery, reflecting both surgical expertise and modern technological support.

At World Laparoscopy Hospital, laparoscopic oophorectomy in pediatric cases is performed following strict safety protocols and evidence-based surgical standards. The hospital is globally recognized for its excellence in minimal access surgery training and advanced laparoscopic procedures. In pediatric gynecological or surgical emergencies—such as ovarian torsion, large ovarian cysts, or non-viable ovarian tissue—laparoscopic oophorectomy may be required to prevent further complications.

The procedure begins with detailed preoperative evaluation including ultrasound imaging, blood investigations, and anesthetic assessment. In a 3-year-old child, anesthesia management is crucial and is handled by experienced pediatric anesthesiologists. Under general anesthesia, small ports (usually 3–5 mm) are inserted into the abdomen to allow introduction of a laparoscope and specialized pediatric instruments. Carbon dioxide is gently insufflated to create working space while maintaining safe pressure limits suitable for pediatric patients.

During surgery, the affected ovary is carefully identified. Surrounding structures such as the fallopian tube, uterus, and bowel are preserved with utmost care. Advanced energy devices help in precise dissection and safe sealing of blood vessels, minimizing blood loss. Once detached, the ovary is retrieved through a small port site, maintaining minimal tissue trauma.

The advantages of laparoscopic surgery in such young patients are significant. These include smaller incisions, reduced postoperative pain, faster recovery, shorter hospital stay, and minimal scarring. Children undergoing laparoscopic procedures typically resume normal activities much faster compared to open surgery.

Postoperative care focuses on pain management, early feeding, and monitoring for complications. Most pediatric patients recover quickly and can be discharged within 24–48 hours depending on the clinical condition. Parents are counseled regarding wound care, activity restriction, and follow-up schedules.

The successful performance of laparoscopic oophorectomy in a 3-year-old child demonstrates the advancement of pediatric minimally invasive surgery and highlights the importance of specialized centers with expert surgical teams and advanced infrastructure.
