This video demonstrates Laparoscopic Surgery for a Large Intramural Fibroid Uterus by Dr. R.K. Mishra at World Laparoscopy Hospital. Uterine leiomyomas (myomas/fibroid) are benign smooth muscle tumors arising from the myometrium. Most myomas do not cause clinical symptoms and do not require intervention. Nonetheless, the size and location of a myoma are important determinants of its potential to become symptomatic and cause problems ranging from infertility to life-threatening uterine hemorrhage.

Leiomyomas may develop anywhere in the myometrium and occasionally in the cervix, broad ligament, and ovaries. Most frequently, they develop in the myometrial wall and can lead to uterine distortion of both the cavity and the overall contour of the uterus, if large and multiple.

Historically, symptomatic myomas have been treated surgically, often by hysterectomy. The surgical management of myomas has advanced significantly with newer, less-invasive forms of therapy. Current options for the management of myomas are numerous and allow for the individualized treatment depending on the patient’s desires. Despite these advances, there still are considerable controversies and unanswered questions about optimal surgical or medical management of symptomatic and asymptomatic myomas among physicians.

Laparoscopic Surgery for Large Intramural Fibroid Uterus by Dr. R. K. Mishra at World Laparoscopy Hospital

Laparoscopic surgery has revolutionized the field of gynecology by offering safer, minimally invasive solutions for complex uterine conditions. One of the most challenging situations for gynecologic surgeons is the management of a large intramural fibroid uterus. Intramural fibroids, which develop within the muscular wall of the uterus, can grow significantly in size and cause symptoms such as heavy menstrual bleeding, pelvic pain, infertility, and pressure on surrounding organs. At World Laparoscopy Hospital, Dr. R. K. Mishra has demonstrated advanced expertise in performing laparoscopic surgery for large intramural fibroids, providing patients with effective treatment and faster recovery.

Large intramural fibroids often present surgical challenges due to their size, location, and vascularity. Traditionally, such cases were managed through open abdominal surgery, which required large incisions and long recovery periods. However, with the advancement of laparoscopic techniques, surgeons can now remove large fibroids through small incisions using specialized instruments and high-definition cameras. Dr. Mishra’s approach focuses on precision, patient safety, and the preservation of uterine function whenever possible.

The laparoscopic procedure begins with careful patient evaluation, including imaging studies such as ultrasound or MRI to determine the exact size and location of the fibroid. During surgery, small incisions are made in the abdomen through which a laparoscope and surgical instruments are inserted. The laparoscope provides a magnified view of the pelvic organs, allowing the surgeon to operate with exceptional accuracy. The fibroid is carefully dissected from the uterine wall, controlling bleeding with advanced energy devices and suturing techniques.

One of the key advantages of laparoscopic myomectomy performed by experienced surgeons like Dr. Mishra is the minimal trauma to surrounding tissues. The uterus is reconstructed using intracorporeal suturing to maintain its structural integrity, which is particularly important for women who wish to preserve fertility. After removal, the fibroid tissue may be extracted using morcellation techniques or through a small incision, depending on the case.

Patients undergoing laparoscopic surgery for large intramural fibroids at World Laparoscopy Hospital benefit from several advantages compared to traditional open surgery. These include smaller scars, reduced postoperative pain, shorter hospital stay, minimal blood loss, and a quicker return to normal activities. In addition, the magnified laparoscopic view allows for better visualization of pelvic anatomy, reducing the risk of complications.

Dr. R. K. Mishra’s contributions to laparoscopic surgery extend beyond patient care. As an internationally recognized laparoscopic surgeon and educator, he trains doctors from around the world at World Laparoscopy Hospital in advanced minimally invasive surgical techniques. Through live surgical demonstrations, structured training programs, and hands-on workshops, surgeons gain valuable experience in managing complex cases such as large intramural fibroids laparoscopically.

In conclusion, laparoscopic surgery for large intramural fibroid uterus represents a significant advancement in modern gynecological treatment. With expert surgeons like Dr. R. K. Mishra leading the way at World Laparoscopy Hospital, patients can benefit from safe, effective, and minimally invasive solutions for complex uterine conditions. The combination of surgical skill, advanced technology, and dedicated training ensures improved outcomes and a higher standard of care for women worldwide.

This procedure is considered when the largest fibroid is less than 10cm in size and the total no of fibroids is less than 5. However, sometimes larger and more numerous fibroids can be removed by this route depending on individual circumstances. Generally, if estimated duration of surgery is expected to be less than three hours, a laparoscopic approach is used. Sometimes, it can also be used as a two or three stage procedure.
