This video demonstrates the surgical technique of Laparoscopic Myomectomy for Large. Intramural Myoma. Myomectomy (my-o-MEK-to-me) is a surgical procedure to remove uterine fibroids also called leiomyomas (lie-o-my-O-muhs). These common noncancerous growths appear in the uterus. Uterine fibroids usually develop during childbearing years, but they can occur at any age. Compared with women who have a laparotomy, women who undergo laparoscopy have less blood loss, shorter hospital stays and recovery, and lower rates of complications and adhesion formation after surgery. There are limited comparisons between laparoscopic and robotic myomectomy. Robotic surgery may take longer and be more costly, but otherwise few differences in outcomes are reported.

Laparoscopic myomectomy is a minimally invasive surgical procedure used to remove uterine fibroids while preserving the uterus. It has become the preferred method for women with intramural fibroids, particularly those who wish to maintain fertility or avoid a major abdominal surgery.

What is an Intramural Fibroid?

Intramural fibroids are the most common type of uterine fibroid. They develop within the muscular wall of the uterus and can cause:

Heavy or prolonged menstrual bleeding

Pelvic pain or pressure

Infertility or recurrent pregnancy loss

Frequent urination or bloating

Unlike submucosal or subserosal fibroids, intramural fibroids remain embedded within the uterine wall, which can make traditional surgery more challenging.

Why Laparoscopic Myomectomy?

Laparoscopic myomectomy is a minimally invasive procedure that involves small incisions in the abdomen through which specialized instruments and a camera are inserted. Advantages include:

Smaller incisions → minimal scarring

Reduced pain → faster recovery

Shorter hospital stay → most patients go home the same day or within 24 hours

Fertility preservation → uterus remains intact

The Procedure

Anesthesia: General anesthesia is administered to ensure comfort.

Incisions: Small incisions (usually 0.5–1 cm) are made in the abdomen.

Fibroid Removal: A laparoscope (camera) provides a magnified view, and surgical instruments carefully remove the intramural fibroid.

Uterine Repair: The uterine wall is meticulously stitched to restore normal anatomy.

Fibroid Extraction: The fibroid tissue is removed, often using a morcellator to break it into smaller pieces.

The entire procedure typically takes 1–3 hours depending on the size and number of fibroids.

Recovery

After laparoscopic myomectomy:

Most patients resume normal activities within 1–2 weeks.

Mild pain and discomfort at incision sites are common.

Avoid heavy lifting and strenuous activity for 4–6 weeks.

Follow-up scans may be recommended to ensure complete removal.

Risks and Considerations

Although laparoscopic myomectomy is generally safe, some risks include:

Bleeding or infection

Adhesion formation

Rare injury to surrounding organs

Fibroid recurrence (in some cases)
