This video is lecture of Dr R K Mishra on Laparoscopic Appendectomy. Laparoscopic Appendectomy can be performed by single incision, two port or three incisions, one umbilical and one suprapubic are made; permanent material used comprises: grasping forceps, hook, scissors, needle holders, three metal trocars and four other usual instruments, and a single strand of cotton. There is possibility of using endobag and in few cases no need to use of operative extractors bags, clips, endoloops, staples or bipolar or harmonic energy instruments. Allows triangulation and instrumentation in the conventional manner.

Acute appendicitis is one of the most common surgical emergencies worldwide. With advancements in minimally invasive surgery, laparoscopic appendectomy has emerged as a preferred approach due to its benefits over conventional open surgery. A lecture on this topic provides medical students, surgical residents, and practicing surgeons with insights into the procedure, indications, techniques, and post-operative care.

Introduction to Acute Appendicitis

Acute appendicitis is an inflammation of the appendix, often caused by obstruction of its lumen, leading to bacterial overgrowth and infection. Patients typically present with right lower quadrant abdominal pain, nausea, vomiting, and fever. Timely diagnosis and intervention are critical to prevent complications such as perforation or peritonitis.

Evolution of Laparoscopic Appendectomy

Since its first description in the late 1980s, laparoscopic appendectomy has revolutionized the treatment of acute appendicitis. Unlike open appendectomy, laparoscopic surgery involves small incisions, a camera (laparoscope), and specialized instruments. This approach provides better visualization of the abdominal cavity, making it especially useful in cases with atypical presentations or uncertain diagnoses.

Indications

Laparoscopic appendectomy is indicated for:

  • Acute uncomplicated appendicitis

  • Complicated appendicitis with localized abscess (in selected cases)

  • Patients requiring diagnostic laparoscopy due to atypical symptoms

It may also be preferred in obese patients and women of reproductive age to rule out gynecological pathologies.

Surgical Technique

  1. Patient Positioning and Anesthesia

    • The patient is placed in a supine position under general anesthesia.

    • A slight Trendelenburg and left tilt may help improve visualization.

  2. Port Placement

    • Typically, three ports are used:

      • Umbilical port for the laparoscope

      • Two additional ports in the lower abdomen for working instruments

  3. Exploration and Identification

    • The abdominal cavity is insufflated with CO₂.

    • The appendix is identified, its base examined, and surrounding structures assessed.

  4. Dissection and Appendiceal Ligation

    • The mesoappendix is carefully dissected and cauterized.

    • The base of the appendix is ligated with endoloops or staplers.

    • The appendix is removed through the umbilical port, often using a retrieval bag to avoid contamination.

  5. Closure and Recovery

    • Ports are removed, pneumoperitoneum released, and small incisions are closed.

    • Postoperative recovery is generally faster, with reduced pain and earlier return to normal activity compared to open surgery.

  • Lower risk of wound infection

  • Faster return to daily activities

  • Enhanced diagnostic capability in uncertain cases

Postoperative Care

Postoperative management includes pain control, early ambulation, and gradual resumption of diet. Antibiotics may be continued if there was perforation or abscess. Patients are monitored for complications like intra-abdominal abscess or port-site infection.
