This video demonstrate Laparoscopic Cholecystectomy with Laparoscopic Ovarian Drilling in same patient by Dr R K Mishra at World Laparoscopy Hospital. Laparoscopic ovarian drilling is a surgical treatment that can trigger ovulation in women who have polycystic ovary syndrome (PCOS). Electrocautery or a laser is used to destroy parts of the ovaries. A laparoscopic cholecystectomy is a surgery during which the doctor removes your gallbladder. This procedure uses several small cuts instead of one large one. A laparoscope, a narrow tube with a camera, is inserted through one incision. This allows surgeon to see gallbladder on a HD monitor. Laparoscopic cholecystectomy is Gold standard for Gallstone disease.

In modern medicine, minimally invasive surgeries have revolutionized patient care by reducing recovery time, minimizing complications, and improving surgical precision. Among these advancements, laparoscopic procedures have become the gold standard for several abdominal and pelvic conditions. Two such procedures—Laparoscopic Cholecystectomy (LC) and Laparoscopic Ovarian Drilling (LOD)—can sometimes be performed together in select patients, offering a safe and efficient approach.

Understanding Laparoscopic Cholecystectomy (LC)

Laparoscopic cholecystectomy is a minimally invasive procedure to remove the gallbladder, commonly indicated for patients suffering from gallstones, cholecystitis, or biliary dyskinesia.

Procedure Overview:

  • Performed under general anesthesia.

  • Small incisions are made in the abdomen.

  • A laparoscope (tiny camera) and specialized instruments are inserted.

  • The gallbladder is carefully dissected and removed.

  • The incisions are closed with minimal scarring.

Benefits of LC:

  • Reduced post-operative pain.

  • Faster recovery compared to open surgery.

  • Shorter hospital stay.

  • Lower risk of infections and complications.

Understanding Laparoscopic Ovarian Drilling (LOD)

Laparoscopic ovarian drilling is a minimally invasive surgery used to treat women with Polycystic Ovary Syndrome (PCOS) who have not responded to medical treatments for ovulation induction. It helps restore ovulation and improve fertility.

Procedure Overview:

  • Small laparoscopic incisions in the abdomen.

  • A laparoscope is used to visualize the ovaries.

  • Using a specialized instrument, small holes are made in the ovarian surface to reduce androgen production.

  • This helps stimulate regular ovulation cycles.

Benefits of LOD:

  • Minimally invasive with rapid recovery.

  • Reduces excessive male hormone (androgen) levels.

  • Improves chances of conception.

  • Avoids prolonged hormone therapy.

Combining LC and LOD: When and Why?

In certain patients, women may present with coexisting gallbladder disease and PCOS-related infertility. Performing LC and LOD together is feasible and beneficial in such cases:

  1. Single Anesthesia: Patients undergo only one general anesthesia session.

  2. Reduced Hospital Visits: One combined surgery reduces multiple hospital admissions.

  3. Faster Overall Recovery: Post-operative downtime is shortened.

  4. Comprehensive Care: Addresses both abdominal and reproductive concerns simultaneously.

Considerations for Combined Surgery:

  • Careful patient selection is crucial; both conditions must require surgical intervention.

  • Surgeons must have expertise in both upper abdominal and gynecologic laparoscopic procedures.

  • Post-operative monitoring is essential to manage pain, prevent infection, and ensure proper healing.

Recovery and Outcomes

Recovery from combined LC and LOD is generally smooth, with most patients:

  • Experiencing mild pain managed with oral analgesics.

  • Resuming normal activities within 1–2 weeks.

  • Reporting improved fertility outcomes if LOD was performed for PCOS.

  • Maintaining long-term benefits from gallbladder removal, including resolution of biliary pain and prevention of complications.

Conclusion

The combination of Laparoscopic Cholecystectomy with Laparoscopic Ovarian Drilling represents an innovative, patient-friendly approach to managing coexisting abdominal and reproductive disorders. By leveraging minimally invasive techniques, surgeons can provide effective treatment with less pain, shorter recovery, and better overall outcomes. This approach highlights the power of laparoscopic surgery in delivering comprehensive care without compromising patient comfort or safety.
