This video demonstrates Right-Sided IPOM Inguinal Hernia Repair with Left-Sided Varicocelectomy performed by Dr. R K Mishra at World Laparoscopy Hospital. Laparoscopic IPOM repair for inguinal hernia is a standard procedure but is occasionally performed in patients who are good candidates for the repair of intraperitoneal Onlay mesh repair due to a peritoneal defect of small size or the presence of a fibrotic and scarred preperitoneal cavity. 

What is IPOM hernia repair?

The IPOM (Intraperitoneal Onlay Mesh) technique is a special repair procedure where a mesh is introduced into the abdominal cavity and placed from the inside over the hernia opening. To conduct this endoscopic IPOM technique, a small incision is first made in the scar-free abdominal wall.

Laparoscopic varicocelectomy is efficient in the treatment of symptomatic varicoceles with a low complication rate. However, careful patient selection is necessary because it appears that individuals presenting with sharp, radiating testicular pain and/or a low-grade varicocele are less likely to benefit from this procedure. Laparoscopic varicocelectomy is a valid therapeutic approach to improve semen parameters for further assisted reproductive techniques. Spermatogenesis may be induced in patients with NOA. Normalization of semen parameters can be achieved for patients with OAT.

The coexistence of inguinal hernia and varicocele is not uncommon in adult male patients. With advancements in minimally invasive surgery, both conditions can be effectively managed in a single laparoscopic session, reducing hospital stay, postoperative pain, and recovery time. This article discusses the combined surgical approach of Right-Sided Intraperitoneal Onlay Mesh (IPOM) Inguinal Hernia Repair along with Left-Sided Laparoscopic Varicocelectomy.

Clinical Background

A right-sided inguinal hernia occurs when abdominal contents protrude through a weakness in the inguinal region, often causing groin discomfort or swelling. A left-sided varicocele, caused by dilated pampiniform venous plexus, is more common due to anatomical factors and is frequently associated with scrotal pain and male infertility.

Performing both procedures laparoscopically offers excellent visualization, precise dissection, and minimal tissue trauma.

Preoperative Evaluation

Detailed clinical examination

Ultrasonography of the groin and scrotum

Semen analysis (when infertility is a concern)

Routine pre-anesthetic investigations

Proper patient selection and counseling are essential before planning combined laparoscopic surgery.

Surgical Technique
1. Right-Sided IPOM Inguinal Hernia Repair

Pneumoperitoneum is established using a standard laparoscopic approach.

Hernial defect is identified and contents reduced.

A composite mesh is placed intraperitoneally covering the myopectineal orifice.

Mesh fixation is achieved using tackers or sutures, ensuring adequate overlap to prevent recurrence.

2. Left-Sided Laparoscopic Varicocelectomy

The left spermatic vessels are identified at the level of the internal ring.

Dilated testicular veins are carefully isolated and clipped or coagulated while preserving the testicular artery and lymphatics.

This approach reduces postoperative hydrocele formation and improves testicular function.

Outcomes and Prognosis

Combined laparoscopic Right-Sided IPOM Hernia Repair with Left-Sided Varicocelectomy has shown excellent outcomes with low recurrence rates and significant improvement in symptoms. In patients treated for infertility, improvement in semen parameters is commonly observed over time.

Conclusion

The simultaneous laparoscopic management of inguinal hernia and varicocele is a safe, effective, and patient-friendly approach. With proper surgical expertise and adherence to standard techniques, this combined procedure offers optimal results with minimal morbidity, making it an ideal option in modern minimally invasive surgery.
