Gen Laparoscopic Surgery / Jul 26th, 2018 5:31 am     A+ | a-

This video demonstrate Laparoscopic cardiomyotomy is the treatment of choice for patients with achalasia of the esophagus. Several different techniques and modifications have been reported concerning the approach (thoracoscopic or laparoscopic), type and length of the myotomy, with or without fundoplication, type of fundoplication. we report our simplified technique for anterior cardiomyotomy with Dor fundoplication and the results obtained using this procedure.

Laparoscopic Cardiomyotomy and Dor Fundoplication at World Laparoscopy Hospital

Laparoscopic cardiomyotomy with Dor fundoplication is a highly effective surgical procedure for the treatment of achalasia cardia and other esophageal motility disorders. At World Laparoscopy Hospital (WLH), this advanced minimally invasive surgery is performed with precision, combining modern technology, expert surgical skills, and structured training. The procedure not only relieves dysphagia but also restores quality of life for patients suffering from chronic swallowing difficulties.

Achalasia is a condition characterized by the failure of the lower esophageal sphincter (LES) to relax properly, leading to progressive dysphagia, regurgitation, chest pain, and weight loss. Laparoscopic cardiomyotomy, commonly known as Heller’s myotomy, addresses this problem by carefully dividing the muscle fibers of the lower esophagus and upper stomach. This releases the functional obstruction and allows food to pass smoothly into the stomach. However, cutting the sphincter muscle can predispose patients to gastroesophageal reflux. To prevent this, Dor fundoplication—an anterior 180–200° partial wrap of the stomach around the esophagus—is performed simultaneously to protect against reflux while maintaining esophageal emptying.

At World Laparoscopy Hospital, the procedure begins with precise port placement under general anesthesia. A high-definition laparoscopic camera provides magnified visualization of the esophagogastric junction. The surgeon meticulously dissects the esophageal hiatus and identifies the lower esophageal sphincter. The myotomy is performed with careful separation of muscle fibers without injuring the underlying mucosa. Once an adequate length of myotomy is achieved, Dor fundoplication is constructed by suturing the anterior gastric wall over the exposed esophagus. This reinforces the mucosa and significantly reduces postoperative reflux.

One of the key advantages of performing this surgery laparoscopically at WLH is minimal postoperative pain, reduced hospital stay, early oral intake, and faster recovery compared to open surgery. The magnified view provided by laparoscopy enhances surgical precision and reduces complications such as mucosal perforation. Patients typically experience immediate relief from dysphagia and show excellent long-term outcomes.

World Laparoscopy Hospital is internationally recognized for its excellence in minimal access surgery. Surgeons and gynecologists from across the globe receive hands-on training in advanced laparoscopic procedures, including cardiomyotomy and fundoplication. Under the leadership of experienced faculty, participants observe live surgeries, practice on simulators, and understand the step-by-step technique with emphasis on safety and evidence-based practice. The hospital’s commitment to innovation and education ensures that both patients and trainees benefit from the highest standards of care.

In conclusion, laparoscopic cardiomyotomy with Dor fundoplication at World Laparoscopy Hospital represents the gold standard treatment for achalasia. With its combination of advanced technology, skilled surgeons, and structured training programs, WLH continues to set benchmarks in minimally invasive esophageal surgery. The procedure not only alleviates debilitating symptoms but also reflects the hospital’s dedication to surgical excellence and patient-centered care.
