Watch this informative video on Diagnostic Laparoscopy, Ovarian Drilling for PCOD, and Tubal Patency Test. Learn how these minimally invasive procedures help in diagnosing and treating infertility, improving ovulation, and assessing fallopian tube health.

This video explains step-by-step procedures, benefits, and what to expect during recovery. Ideal for patients and medical students seeking clear insights into gynecological laparoscopy.

Diagnostic Laparoscopy Ovarian Drilling for PCOD and Tubal Patency Test thus not only helps in regulating ovulation and enhancing conception rates but also provides an opportunity to assess the pelvis for other potential causes of subfertility which could be treated at the same time. We therefore believe that diagnostic hysteroscopy and laparoscopy should be offered quite high-up in the hierarchy of infertility investigations and treatment. PCOS is characterized by presence of the hyperandrogenism, oligo- or anovulation. It is one of the most common reproductive endocrine disorders in the adolescent women. Females with PCOS present with a dysregulation in the menstrual cycle, hirsutism, anovulation, acne, and obesity. An approximately 5-10% of women in Poland suffer from PCOS and 10-15% of them are infertile.

Infertility affects a significant number of couples worldwide, with conditions like Polycystic Ovary Syndrome (PCOS/PCOD) and tubal blockages being major contributors. Modern gynecological procedures, such as diagnostic laparoscopy, ovarian drilling, and tubal patency testing, play a crucial role in both diagnosis and treatment, offering hope to women struggling with infertility.

What is Diagnostic Laparoscopy?

Diagnostic laparoscopy is a minimally invasive surgical procedure that allows doctors to examine the organs inside the abdomen, particularly the female reproductive system, including the uterus, fallopian tubes, and ovaries. This procedure is performed using a laparoscope, a thin tube with a light and camera, inserted through a small incision in the abdomen.

Purpose of Diagnostic Laparoscopy:

  • Evaluate tubal patency (whether the fallopian tubes are open or blocked)

  • Detect conditions like endometriosis, adhesions, or ovarian cysts

  • Assess the ovarian morphology, especially in women with PCOD

  • Identify unexplained causes of infertility

Ovarian Drilling for PCOD

Polycystic Ovary Disease (PCOD) is a common hormonal disorder causing irregular periods, excessive androgen levels, and multiple ovarian cysts. It can lead to anovulation (lack of ovulation) and infertility.

Laparoscopic ovarian drilling (LOD) is a procedure used to induce ovulation in women with PCOD who do not respond to medications like clomiphene citrate.

How It Works:

  • Small punctures or “drills” are made in the ovaries using laser or electrocautery.

  • These punctures reduce androgen levels and restore normal ovulation cycles.

  • It is a minimally invasive procedure with quick recovery, often allowing natural conception post-treatment.

Benefits:

  • Improves ovulation and fertility

  • Reduces androgen levels

  • Can be an alternative to long-term medication

Tubal Patency Test

Blocked fallopian tubes are a common cause of female infertility. A tubal patency test evaluates whether the fallopian tubes are open and functional.

Methods:

  1. Hysterosalpingography (HSG) – Uses X-ray with contrast dye to visualize the tubes.

  2. Sonohysterography with saline and air bubbles – Ultrasound-based method.

  3. Diagnostic laparoscopy with chromopertubation – Gold standard: a special dye is introduced into the uterus and observed laparoscopically to see if it passes through the tubes.

Advantages of Laparoscopic Approach:

  • Minimally invasive with smaller incisions

  • Direct visualization of pelvic organs

  • Can treat conditions simultaneously (like ovarian drilling or removal of adhesions)

  • Shorter recovery time compared to open surgery

Conclusion

Diagnostic laparoscopy, combined with ovarian drilling and tubal patency testing, is a powerful tool in evaluating and treating infertility. These procedures not only help identify underlying causes but can also provide therapeutic solutions in the same sitting. Women struggling with infertility, especially those with PCOD or suspected tubal blockages, can benefit immensely from consulting experienced laparoscopic surgeons.
