The uterine manipulators in use for various gynecological laparoscopic surgeries including laparoscopic hysterectomy or laparoscopic pelvic endometriosis have to achieve many different tasks in order to arrive at a safe and successful outcome in gynecological and pelvic surgery. The most obvious function of Uterine Manipulators is to suitably mobilize the uterus.

Uterine manipulators are indispensable instruments in modern gynecologic surgery, particularly in minimally invasive procedures such as laparoscopy and robotic-assisted surgery. They play a crucial role in providing optimal exposure, improving surgical precision, and enhancing patient safety during complex pelvic operations.

What Is a Uterine Manipulator?

A uterine manipulator is a surgical device inserted through the cervix into the uterine cavity to mobilize and position the uterus during gynecologic procedures. By allowing controlled movement of the uterus in multiple directions—anteversion, retroversion, lateral displacement, and elevation—the surgeon gains better visualization of pelvic anatomy and improved access to critical structures.

Importance in Laparoscopic and Robotic Surgery

In laparoscopic and robotic gynecologic surgeries, visualization and working space are limited. Uterine manipulators help overcome these challenges by:

  • Enhancing exposure of the uterine vessels, uterosacral ligaments, and vaginal fornices

  • Facilitating safe dissection around the bladder and rectum

  • Improving accuracy during colpotomy in hysterectomy

  • Reducing operative time and surgeon fatigue

Their use is especially valuable in procedures such as total laparoscopic hysterectomy (TLH), laparoscopic-assisted vaginal hysterectomy (LAVH), myomectomy, endometriosis surgery, and pelvic lymphadenectomy.

Types of Uterine Manipulators

Several uterine manipulators are available, each designed for specific surgical needs:

  1. Vaginal Cup Manipulators
    These include a cervical cup that delineates the vaginal fornices, aiding safe colpotomy during hysterectomy. Examples are commonly used in TLH and robotic hysterectomy.

  2. Balloon-Tip Manipulators
    These feature an inflatable balloon within the uterine cavity to provide secure anchorage and controlled uterine movement.

  3. Articulating Manipulators
    Designed with flexible joints, these allow a wide range of uterine motion, making them useful in complex pelvic surgeries.

  4. Disposable vs. Reusable Manipulators
    Disposable manipulators offer convenience and sterility, while reusable ones are cost-effective and commonly used in high-volume surgical centers.

Role in Surgical Safety

Proper use of uterine manipulators significantly enhances surgical safety. By clearly defining anatomical planes, they help reduce the risk of injury to adjacent organs such as the ureters, bladder, and bowel. Accurate placement and gentle manipulation are essential to prevent uterine perforation, cervical trauma, or infection.

Technique and Precautions

Successful use of a uterine manipulator requires:

  • Careful preoperative assessment of uterine size, shape, and pathology

  • Gentle cervical dilation and correct insertion technique

  • Avoidance of excessive force during manipulation

  • Removal under direct visualization at the end of the procedure

In cases of cervical stenosis, large fibroids, or malignancy, alternative strategies or specialized manipulators may be required.

Limitations and Contraindications

While highly beneficial, uterine manipulators may not be suitable in all situations. Contraindications include severe cervical stenosis, advanced cervical or endometrial cancer, active pelvic infection, and certain pregnancy-related conditions. In such cases, surgeons may rely on external manipulation or other laparoscopic techniques.

Future Perspectives

With ongoing advancements in minimally invasive surgery, uterine manipulators continue to evolve. Modern designs focus on improved ergonomics, enhanced articulation, and better integration with robotic platforms, further improving surgical efficiency and patient outcomes.
