This video demonstrate how to use various hysteroscopic instruments. Hysteroscopy is a procedure that allows us to look inside the uterus in order to diagnose and treat causes of abnormal bleeding. The procedure can be either diagnostic or operative. Hysteroscopy is a procedure that allows gynecologist to look inside your uterus in order to diagnose and treat causes of abnormal bleeding. Hysteroscopy is done using a hysteroscope, a thin, lighted tube that is inserted into the vagina to examine the cervix and inside of the uterus. Hysteroscopy can be either diagnostic or operative.

Hysteroscopy is a minimally invasive procedure used to inspect the uterine cavity, diagnose abnormalities, and perform certain surgical interventions. The use of hysteroscopy instruments requires precision, knowledge, and careful handling to ensure patient safety and effective outcomes. This article provides a step-by-step guide on how to use hysteroscopy instruments effectively.

1. Understanding Hysteroscopy Instruments

Hysteroscopy instruments are broadly classified into diagnostic and operative tools:

Diagnostic Instruments:

Hysteroscope: A thin, lighted telescope inserted into the uterus. Available in rigid or flexible types.

Camera System: Connects to the hysteroscope to provide a clear view on a monitor.

Light Source: Provides illumination for proper visualization.

Distension Medium: Saline or CO₂ gas used to expand the uterine cavity.

Operative Instruments:

Scissors and Forceps: For cutting tissue or removing polyps and fibroids.

Resectoscope: A specialized hysteroscope with electrosurgical capabilities.

Graspers and Biopsy Instruments: To collect tissue samples for pathology.

2. Preparing for the Procedure

Proper preparation is essential for safe and effective hysteroscopy:

Patient Preparation: Ensure the patient has been evaluated and consented. Vaginal antisepsis and anesthesia (local, regional, or general) are applied as appropriate.

Instrument Sterilization: All instruments must be thoroughly sterilized before use.

Setup: Connect the hysteroscope to the camera, light source, and fluid management system. Check for leaks and proper function.

3. Using Diagnostic Hysteroscopy Instruments

Step-by-Step Process:

Introduce the Speculum: Gently insert a vaginal speculum to visualize the cervix.

Cervical Dilation: Use a dilator if necessary to allow the hysteroscope to pass through the cervical canal.

Insert the Hysteroscope: Carefully guide the hysteroscope into the uterine cavity.

Distend the Uterus: Infuse saline or CO₂ to expand the cavity for optimal visualization.

Visual Examination: Observe the uterine walls, endometrium, and openings of the fallopian tubes.

4. Using Operative Hysteroscopy Instruments

Operative hysteroscopy allows for intervention during the same procedure:

Polyp or Fibroid Removal: Use graspers or a resectoscope to remove abnormal growths.

Endometrial Biopsy: Insert biopsy forceps through the working channel to collect tissue.

Adhesiolysis: Carefully cut intrauterine adhesions using scissors or electrosurgical tools.

Hemostasis: Ensure any bleeding is controlled using cautery or other hemostatic techniques.

5. Tips for Safe and Effective Use

Always maintain clear visualization; poor visibility increases risks.

Keep distension pressure within safe limits to avoid uterine perforation.

Handle instruments gently and precisely to minimize trauma.

Regularly check the integrity of cables, optics, and instruments.

Ensure proper fluid management to prevent complications from over-distension.

6. Cleaning and Maintenance

After the procedure:

Disassemble all instruments.

Clean thoroughly with enzymatic solutions and brushes.

Sterilize according to manufacturer guidelines.

Store safely to prevent damage to optics and delicate components.

Conclusion

Using hysteroscopy instruments correctly is critical for successful diagnosis and treatment of uterine conditions. Mastery of both diagnostic and operative techniques, along with strict adherence to safety protocols, ensures optimal outcomes for patients. Proper training and regular practice are key for gynecologists and surgical teams to perform hysteroscopy efficiently and safely.
