This video demonstrates laparoscopic inguinal hernia repair by Dr. R K Mishra at World Laparoscopy Hospital. Laparoscopic inguinal hernia repair has got comparable results in comparison to open hernia repair. Many studies have shown that Laparoscopic inguinal hernia repair gives better results in terms of recurrence as compared with open hernia surgery but with the added advantage of fewer chances of post-operative, pain, wound infection, and early return to activity. Laparoscopic inguinal hernia repair was started using the transabdominal preperitoneal (TAPP) approach. Another technique of LIHR that has evolved is totally extraperitoneal (TEP) repair. An inguinal hernia is a bulge that occurs in your groin region, the area between the lower part of your abdomen and your thigh. Inguinal hernias occur because of a weakening of the muscles in the lower abdomen. Three layers protect the intestines inside the lower abdomen. The first is a thin membrane called the peritoneum. The second is made up of the abdominal muscles, and the third is your skin. An inguinal hernia forms when your intestines and the peritoneum push through the muscles and appear as a bulge under your skin. Inguinal hernias are dangerous because they tend to keep getting larger and your intestine can get trapped inside the bulge and lose its blood supply. This is called a strangulated inguinal hernia, and surgery may be needed to correct the problem.

An inguinal hernia occurs when tissue, such as a portion of the intestine, protrudes through a weak spot in the lower abdominal wall, often causing discomfort, pain, or a visible bulge. Among the various types of inguinal hernias, indirect inguinal hernia is the most common, especially in men, and results from a congenital weakness in the abdominal wall.

Understanding TAPP Surgery

TAPP (Transabdominal Preperitoneal) repair is a modern minimally invasive laparoscopic technique for repairing inguinal hernias. Unlike traditional open surgery, TAPP allows surgeons to access the hernia site through small abdominal incisions, offering several advantages:

Reduced postoperative pain

Faster recovery and return to daily activities

Minimal scarring

Lower risk of infection and complications

Step-by-Step Procedure

Anesthesia & Positioning: The patient is placed under general anesthesia, lying on the operating table in a supine position.

Trocar Placement: Small incisions are made to insert laparoscopic instruments and a camera to visualize the hernia from inside the abdomen.

Peritoneal Incision: The peritoneum (lining of the abdominal cavity) is carefully opened to expose the hernia defect.

Hernia Reduction: The protruding tissue or intestine is gently pushed back into the abdominal cavity.

Mesh Placement: A specially designed surgical mesh is positioned over the hernia defect to reinforce the abdominal wall.

Peritoneal Closure: The peritoneum is sutured or closed over the mesh to prevent adhesions and secure the repair.

Completion: Instruments are removed, and small incisions are closed with sutures or surgical glue.

Advantages of TAPP for Indirect Inguinal Hernia

Minimally invasive: Small incisions lead to quicker healing and less postoperative discomfort.

Durable repair: Mesh reinforcement reduces the risk of recurrence.

Bilateral repair: Both sides can be repaired simultaneously if needed, without additional incisions.

Early mobilization: Patients often resume normal activities within a few days.

Recovery and Postoperative Care

Most patients can resume light activity within 1–2 days.

Full recovery and return to strenuous activity usually occur within 2–4 weeks.

Pain and swelling are generally mild and managed with medications.

Maintaining a healthy weight and avoiding heavy lifting can reduce the risk of recurrence.

Conclusion

TAPP repair for indirect inguinal hernia represents a significant advancement in hernia surgery, combining the benefits of laparoscopic precision with effective mesh reinforcement. It ensures minimal discomfort, faster recovery, and long-term durability. Patients suffering from inguinal hernias can experience improved quality of life through this modern surgical approach.

Dr. R.K. Mishra's laparoscopic inguinal hernia repair at World Laparoscopy Hospital is a transformative approach. This technique offers results comparable to traditional open hernia repair but with numerous benefits, including reduced recurrence rates, post-operative pain, wound infections, and faster recovery. The procedure, whether transabdominal preperitoneal (TAPP) or totally extraperitoneal (TEP) repair, effectively addresses the issue of inguinal hernias and prevents complications like strangulation.
