The indications for cholecystectomy remain the same and should not be liberalized  because the laparoscopic procedure is viewed as lower in morbidity  than its open counterpart. Conditions for which the procedure is used include the symptomatic cholelithiasis. Ultrasound con formation of gallstones in conjunction with a classic history is sufficient to make the diagnosis. The most common symptom pattern consists of episodic epigastric or right upper quadrant pain occurring several after meals. Patients with nonspecific symptom, such as nausea, bloating, indication and flatulence, are sometimes benefited by cholecystectomy however, the more the symptoms differ from the classic pattern of biliary pain, the less likely the patterns of biliary pain, the less likely the patient is to experience relief after cholecystectomy.

Laparoscopic appendectomy is indicated when acute appendicitis is suspected or confirmed by computed tomographic (CT) scan .it is especially helpful in the obese patient in a young woman, or when the Diagnosis is in doubt. Laparoscopic removal of the normal appendix is indicated if the indication for the procedure was right lower quadrant pain. Incidental laparoscopic appendectomy is not generally indicated.

For performing appendectomy some surgeons prefer to use the lithotomy position in women. This allows to the perineum so that a cervical manipulator may be use to elevate and provide better visualization of the pelvic organs. We should tuck the patient’s arms at the sides. this is extremely important to allow sufficient room for the assistant and camera operator to move cephalad as ...   Read more...

With the advent of advanced surgical and gynaecological minimally invasive surgery over the last 2 decades, a need has arisen for state of the art training programs dedicated exclusively to these da Vinci Robotic Surgery techniques. None of the medical colleges in Asia has training programs in robotic surgery and surgeons do not get adequate experience in advanced robotic techniques. Approximately 130 MIS training programs have been initiated in the U.S. over the past 15 years. Most programs are heavily weighted in laparoscopic procedures, with none offering experience in da Vinci robotic surgical procedures. A recent published survey completed by fellows in training confirmed that doctors are interested in more complex procedures (Tichansky, et al, Surg Endosc 2007). Some of the major benefits experienced by surgeons using the da Vinci ...   Read more...

Cervical shortening in reproductive age group female is believed to be a marker for generalized intrauterine recurrent inflammation and has a strong association in pregnant women with spontaneous preterm birth. For these patients who has recurrent abortion a variety of therapies, including vaginal and intramuscular progesterone, pessary, and cervical cerclage, have been demonstrated to be effective in specific clinical circumstances.

For these female patients cervical cerclage can be placed via transvaginal, open transabdominal, or laparoscopic transabdominal approach, preferably before pregnancy or during thirteen week of pregnancy. A laparoscopic approach may be superior to the transabdominal approach in terms of surgical outcomes, cost, and postoperative morbidity.

The usage of single-incision laparoscopic surgery may represent a noticeable difference over conventional laparoscopic surgery. In recent more, more and years articles are already published demonstrating the feasibility on this approach. Hence, that is why, we would rather perform this procedure.   Laparoscopic appendectomy has rapidly developed in recent years. Since Semm published the first complete elimination of the appendix via laparoscopic surgery in 1983 and Schreiber performed the first laparoscopic appendectomy in the patient with acute appendicitis in 1987, laparoscopic appendectomy has been a part of practically all hospitals worldwide as the usual procedure in emergency departments.   The tendency toward reduced patient morbidity after surgery has allowed the creation of techniques requiring an ...   Read more...

Laparoscopy is easily the most common procedure employed to remove and diagnose mild to moderate endometriosis. As opposed to employing a large abdominal incision, the surgeon inserts a lighted viewing instrument called a laparoscope using a small incision. If the surgeon needs better access, he or she makes one or two more small incisions for inserting other surgical instruments.   There are several methods for surgically treating an endometrioma, including draining it, removing component of it, or removing it entirely (cystectomy). Any of these treatments brings pain alleviation for almost all women yet not all. Cystectomy is most probably to relieve pain for an extended time, prevent an endometrioma from growing back, and stop the need for another surgery   The laparoscopic surgical procedures are method ...   Read more...

The patient should discuss options together physician to see which procedure is best fitting.   An overall hysterectomy includes the removal of the uterus as well as the cervix. When the ovaries and sometimes the fallopian tubes also are removed, the procedure is known as a hysterectomy having a bilateral oophorectomy.   A good option not often realized by a lot of patients is a supracervical hysterectomy.
