Which application documents must I submit?

Hi! My question is during filling up the online application form which application documents must I submit? It should be notary attested or original photocopy will do? |

Dear Dr Sultana You need to bring the following documents: Application/Registration form: If you've applied via a registration form you may have to produce either the original or an attested photocopy of the form. It is not required if you have applied online. You may bring the printed copy of invitation letter issued by WLH. Photocopy of your identity proof like passport or driving licence. Please also bring your medical qualification certificate, self attested copy along with 2 passport size photograph. Please bring with you payment proof, if you have paid by credit card or wire transfer ( Not necessary for the doctors who has opted for cash on arrival) as a Initial Registration fee. Notary attested certificate is not required. Self attested photocopy of your document is sufficient. With regard Dr J S Chowhan |
