Laparoscopic Repair of Recurrent Umbilical Hernia
This video demonstrates laparoscopic repair of Umbilical recurrent hernia. An umbilical hernia is a rather common surgical problem. Elective repair after diagnosis is advised by the laparoscopic technique. Suture repairs have high recurrence rates; therefore, mesh reinforcement is recommended. Mesh can be placed through either an open or laparoscopic approach with good clinical results. We are performing at World Laparoscopy Hospital for more than 20 years. Laparoscopic umbilical hernia repair has been practiced at World Laparoscopy Hospital since the late 1990s. Newer bilayer prosthetic devices are designed for open intraperitoneal inlay placement. They have two sides, one is polypropylene and the other side is a non-adherent material to face viscera. Bilayer polypropylene or partially absorbable meshes have also been used for umbilical hernias. They comprised one subway and one overlay patch with a connector to eliminate migration. However, clinical outcomes after repairs with these devices have not been widely documented.
Laparoscopic repair of recurrent umbilical hernia represents an advanced surgical approach that offers significant benefits over conventional open techniques, especially in patients who have previously undergone hernia repair and present with recurrence. Recurrent umbilical hernias are technically challenging due to scar tissue formation, altered anatomy, and weakened fascial layers from prior surgeries. A composite mesh is then introduced into the abdominal cavity and placed intraperitoneally to cover the defect with adequate overlap, usually at least 3–5 cm beyond the margins. The mesh is fixed using transfascial sutures and tackers, ensuring strong reinforcement of the abdominal wall. This technique significantly reduces tension on the repair site, which is crucial in preventing further recurrence.
The advantages of laparoscopic repair of recurrent umbilical hernia include smaller incisions, reduced postoperative pain, shorter hospital stay, early return to daily activities, lower wound complication rates, and better cosmetic outcomes. Additionally, the laparoscopic approach allows identification of multiple defects, known as Swiss-cheese defects, which may be missed in open surgery.
