Obesity and limited uterine mobility may also increase surgical complexity. In such cases, identification of vital structures like the ureter, bladder, and major vessels becomes more challenging and increases the risk of complications if not handled carefully.

Preoperative evaluation plays a crucial role in managing difficult TLH. Detailed imaging such as ultrasound or MRI helps in assessing uterine size, location of fibroids, extent of adhesions, and involvement of surrounding organs. Proper patient counseling and preparation are equally important. Surgeons must be prepared with advanced laparoscopic skills, energy devices, and sometimes modified port placement techniques to ensure better visualization and ergonomics.

During difficult TLH, stepwise surgical dissection, meticulous hemostasis, and constant anatomical orientation are essential. Early identification and safeguarding of ureters and bladder significantly reduce complications. In complex cases, techniques like retroperitoneal dissection, debulking of large uterus, or adhesiolysis are often required. The use of advanced energy sources and high-definition imaging systems further enhances surgical safety and efficiency.

Despite the challenges, difficult TLH offers significant benefits compared to open surgery, including less postoperative pain, minimal blood loss, shorter hospital stay, faster recovery, and better cosmetic outcomes. However, these procedures should ideally be performed by surgeons trained in advanced minimal access surgery in well-equipped centers.

With proper patient selection, thorough preparation, and skilled execution, even complex hysterectomy cases can be safely managed laparoscopically, ensuring better patient outcomes and faster return to normal life.

Total Laparoscopic Hysterectomy is currently accepted as a safe, efficient way to manage benign uterine pathology by doctors and patients and is an acceptable alternative to standard abdominal hysterectomy/ In recent days with the popularization and advance of the Total Laparoscopic Hysterectomy, scholars are increasingly exploring and expanding their surgical scope of application. In the past, a large uterus with uterus size above 12 weeks of gestation was considered as the contraindication for laparoscopic hysterectomy due to limited visibility and access to uterine vascular associated with the high risk of complications such as hemorrhage, bowel and urinary injury, difficulty in extracting the uterus, and extended duration of the procedure and was more suitable for laparotomy. With the expansion of the surgical indications of the laparoscopic hysterectomy, there have been a number of publications reporting the Total Laparoscopic Hysterectomy for large uterus.
