What is TAPP Hernia Repair?

Key Steps in TAPP Hernia Repair

Port Placement and Pneumoperitoneum
Typically involves three ports: one 10 mm camera port at the umbilicus and two 5 mm working ports.
CO₂ insufflation is used to create adequate working space.
Peritoneal Incision
A curvilinear incision is made in the peritoneum 2–3 cm above the hernia defect to enter the preperitoneal space.

Dissection and Identification

Gentle dissection is performed to expose anatomical landmarks including the inferior epigastric vessels, spermatic cord, vas deferens, and herniated sac.
The hernia sac is carefully reduced, and surrounding tissues are cleared for mesh placement.

Mesh Placement

A large synthetic mesh (typically polypropylene or 3D mesh) is introduced and positioned to cover the entire myopectineal orifice, extending beyond the boundaries of the defect.

Peritoneal Closure

The peritoneum is closed using continuous or interrupted sutures or tackers to isolate the mesh from intra-abdominal contents, preventing adhesions or bowel complications.

Advantages of TAPP Technique

Excellent anatomical visualization of the groin structures.
Simultaneous repair of bilateral hernias through the same ports.
Effective in recurrent hernias where anterior repair has failed.
Less postoperative pain and faster recovery compared to open techniques.
Low recurrence rate due to complete coverage of the defect area with large mesh.
Enhanced cosmetic outcome due to small incisions.

Clinical Considerations and Contraindications

While TAPP is a highly effective approach, it should be performed by surgeons trained in laparoscopic hernia repair. Caution is required in patients with prior extensive lower abdominal surgery, dense adhesions, or contraindications to general anesthesia. In such cases, open or TEP (Totally Extraperitoneal) approaches may be more appropriate.
