Hiatal hernia post lap. sleeve gastrectomy

Dear sir,
I have one patient , post lap sleeve gastrectomy one month ago, came to emergency department complaining of epigastric pain and retching. In CT there is wide hiatal hernia , rolling. What to do in this situation? thanks

During Sleeve Gastrectomy we should always check for hiatus hernia. It crural approximation of hiatus hernia is not done during sleeve gastrectomy then severe infarction of the stomach also can happen due to sliding of the stomach in the mediastinum. This patient should be operated on immediately. After laparoscopic sleeve gastrectomy intractable vomiting to include post sleeve hiatal hernia with a potential risk of strangulation. Emphasis should be on including thoracic cuts in the CT scan which will diagnose the presence of surgical staples inside the thorax.
