Dear Dr Vida Heparinized saline is use to aid aspiration of blood clots during laparoscopy wherever there is hemoperitoneum with clots. 5000 International Unit of Heparin should be mixed with 1 litre of saline and it should be irrigated over the clot. With the use of the larger pool-tip suction cannulas the clot should be beaten and by this technique it will break in small peaces making clot removal easily. A warmed isotonic solution of normal saline or lactated Ringer's can also be used to mix with heparin. Direct application of suction tubing to remove the clot without heparin is possible but often cause soft tissue trauma and the tubing itself can also become blocked by soft-tissue debris thereby increasing surgical time.
