I ask about the lever type 2 and type 3 is of concern in laparoscopic surgery because the movement and the different angle affect the performance. Is it of concern in robotic surgery?

Dear Dr Elssayed

Your question is very intelligent. In da Vinci Robotic Surgery type 2 and type 3 lever action does not create any problem as software is compensating this leverage changes. However changing to type II to type III lever will change the elevation angle and that can create a big problem in robotic surgery. With wrong elevation angles arm can hit each other and arm can also have restricted movement if it touch the body of the patient.

This is the reason even in da Vinci robotic surgery we should apply baseball diamond concept and half of the instrument should be in and half should be out so that elevation angle will remain around 30 degree.

Robot need more correct ergonomics than human and docking of the daVinci with correct port position is one of the important task of a good robotic surgeon.
