Difficulty in Laparoscopic Cholecystectomy

I want your help if u can give me some... i am a general surgeon and i did my first laparoscopic cholecystectomy 2 days ago... the patient is well but i had two major problems during the operation... one problem was my view of the peritoneal cavity was seriously limited even though the intraabdominal preasure was 12 mmhg and second problem that the stomach was seriously inflated causing me vision problems even though the anesthesiologist tried but failed to aspirate it... can u suggest what can i do in my next case so i wont have those problems? in my residency as a surgeon i watched over 300 laparoscopic cholecystectomies and performed 80 as a surgeon... but now i am in a hospital that no one else is experienced in this subject... i think that the patient did not received enough muochalassis during the operation and that the anaesthesiologist should be able to aspirate the stomach.,.. but i would really care for your opinion,, thank you in advance,,,

Dear Dr Abdullah

Your problem itself id explaining that you are not trained in our institute. Doing surgery is one thing and understanding the basics of ergonomics, task analysis and engineering is another important thing. You have problem with the troubleshooting of laparoscopic ergonomic challenges. To see and advice you what was the actual problem it would be better if you can send the video of your surgery. After seeing it we can advice you what you should do from next tme to increase your visibility.
