I have folded foot from the birth. I want to apply for Job and I want to avail the Handicapped quota. want to know How can I get this certificate.

The Disabled Certificate is being issued under Person With Disabilities Act 1995 (amended 2009). Under this act Disability Certificates are issued by three-member Medical Boards constituted at the District level,The applicant has to fill an application form, submit the required address proof, age proof,and has to pay the required fee along with All the old relevant treatment record.
For filling the form you have to approach the concerned government hospital of you district.
