This video demonstrates Total Laparoscopic Hysterectomy for Very Large Uterus with Huge Fibroid. It is not uncommon for a laparoscopic surgeon to be able to perform the entire laparoscopic hysterectomy using a few 10mm and  5-mm ports and then a larger abdominal incision to remove the surgical specimen using morcellator. In conclusion, this video shows that the size of the uterus does not forbid the laparoscopic approach for hysterectomy. 
 

Total Laparoscopic Hysterectomy for Very Large Uterus with Huge Fibroid

Total Laparoscopic Hysterectomy (TLH) has become one of the most advanced and preferred surgical techniques for the treatment of various uterine conditions, including large fibroids. Traditionally, very large uteri with huge fibroids were managed through open abdominal surgery due to the technical challenges involved. However, with advancements in minimal access surgery, improved surgical instruments, and enhanced laparoscopic skills, TLH is now successfully performed even in cases involving very large uteri.

Uterine fibroids, also known as leiomyomas, are benign tumors that arise from the smooth muscle cells of the uterus. While many fibroids remain small and asymptomatic, some can grow significantly, leading to symptoms such as heavy menstrual bleeding, pelvic pain, abdominal distension, urinary frequency, and reproductive problems. When fibroids become very large and cause severe symptoms, surgical removal of the uterus may become necessary, especially in women who have completed childbearing.

Performing TLH for a very large uterus with a huge fibroid is technically demanding and requires a high level of surgical expertise. One of the main challenges is limited space within the abdominal cavity due to the enlarged uterus. This can make trocar placement, visualization, and instrument manipulation more difficult. Surgeons must carefully plan port placement to ensure optimal access and maintain adequate working space during the procedure.

Another important challenge is controlling blood loss. Large fibroids are often associated with increased vascularity, which increases the risk of bleeding during surgery. Skilled laparoscopic surgeons use advanced energy devices and precise surgical techniques to carefully seal and divide blood vessels while maintaining a clear operative field. Proper identification and protection of surrounding structures such as the ureters, bladder, and bowel are also essential for patient safety.

During the procedure, the uterus is detached laparoscopically from its supporting ligaments and vascular supply. In cases where the uterus is extremely large, techniques such as morcellation or specimen debulking may be used to facilitate removal through minimally invasive access points. These methods allow the surgeon to remove large tissue masses without the need for a large abdominal incision.

Despite the complexity of the procedure, TLH offers numerous benefits compared to open surgery. Patients typically experience less postoperative pain, minimal blood loss, smaller scars, shorter hospital stays, and faster recovery times. Additionally, the magnified view provided by laparoscopic equipment allows for greater precision during surgery, which can improve overall surgical outcomes.

The success of TLH in cases involving very large uteri with huge fibroids largely depends on the experience and skill of the surgical team. Proper patient selection, preoperative imaging, and careful surgical planning are crucial factors that contribute to a safe and effective procedure.

In conclusion, Total Laparoscopic Hysterectomy for a very large uterus with a huge fibroid demonstrates the remarkable progress in minimally invasive gynecological surgery. What was once considered possible only through open surgery can now be performed laparoscopically by experienced surgeons. This advancement not only improves patient outcomes but also highlights the continuous evolution of surgical techniques in modern medicine.
