Dr. R.K. Mishra's lecture on laparoscopic Cervical Cerclage for management of cervical incompetence at CAMLS, University of South Florida, United States of America. This lecture was delivered during the Fellowship of minimal access surgery course of the World Laparoscopy Training Institute, Florida USA.

Laparoscopic Cervical cerclage is a surgical procedure in which your doctor places a single stitch around your cervix, the opening to your uterus. The stitch sews your cervix closed. Doctors perform cerclage to prevent late (second trimester) miscarriages and preterm (early) delivery.

Advancements in minimally invasive surgery have transformed the field of gynecology, offering safer procedures, faster recovery, and improved outcomes for patients. One such remarkable development is laparoscopic cervical cerclage, a procedure used to manage cervical incompetence in women who suffer from recurrent pregnancy loss or premature delivery. A significant contribution to the global understanding of this procedure came through the lecture delivered by Dr. R. K. Mishra in Florida, United States, where he shared his expertise and surgical insights with an international audience of surgeons and gynecologists.

Cervical incompetence is a condition in which the cervix weakens and opens prematurely during pregnancy, often leading to miscarriage or preterm birth. Traditionally, cervical cerclage was performed through a transvaginal approach. However, in complex cases where vaginal cerclage fails or is not feasible, laparoscopic cervical cerclage offers an advanced and effective alternative. During his lecture, Dr. Mishra explained the importance of accurate patient selection, preoperative evaluation, and precise surgical technique to achieve optimal outcomes.

Dr. Mishra emphasized that laparoscopic cervical cerclage allows surgeons to place a strong suture around the cervix at the level of the internal os through minimally invasive access. Using laparoscopic instruments and high-definition visualization, the surgeon can carefully dissect the surrounding structures, identify the uterine vessels, and place the cerclage tape with high precision. This approach not only minimizes surgical trauma but also significantly reduces postoperative pain and recovery time.

During the session in Florida, Dr. Mishra presented detailed surgical videos and step-by-step explanations of the procedure. He highlighted key anatomical landmarks, trocar placement strategies, and techniques to avoid complications. The lecture also included discussions on the timing of the procedure, which is often performed before pregnancy or in early pregnancy when indicated. His presentation helped attending surgeons understand the technical nuances that are crucial for successful laparoscopic cerclage.

A major focus of the lecture was the advantages of the laparoscopic approach compared with traditional open surgery. Dr. Mishra explained that laparoscopy provides better visualization of pelvic anatomy, less blood loss, smaller incisions, and quicker patient recovery. Additionally, the cerclage placed laparoscopically can remain in place for future pregnancies, providing long-term benefits to patients who have experienced repeated pregnancy loss due to cervical insufficiency.

The audience in Florida greatly appreciated Dr. Mishra’s depth of knowledge and his commitment to surgical education. As the founder of World Laparoscopy Hospital, he has trained thousands of surgeons and gynecologists from across the globe in minimal access surgery. His lecture not only demonstrated advanced surgical techniques but also reinforced the importance of continuous learning and innovation in modern medicine.

In conclusion, Dr. Mishra’s lecture on laparoscopic cervical cerclage in Florida served as an important educational event for the international medical community. By sharing his experience and refined surgical techniques, he helped expand the understanding of this life-changing procedure. His dedication to teaching and advancing minimally invasive surgery continues to inspire surgeons worldwide and contributes significantly to improving maternal and fetal outcomes.
