Dear Jiya

A hysterosalpingogram (HSG) is not a very reliable test it is definitely an X-ray test that appears in the inside of the uterus and fallopian tubes and also the area surrounding them.

Many times, it is done for women who are having a difficult time becoming pregnant. Throughout a hysterosalpingogram, a dye contrast material is put through a thin tube that is subjected to the vagina and into the uterus.

Because the uterus and the fallopian tubes are hooked together, the dye will flow in to the fallopian tubes. Pictures are taken utilizing a steady beam of X-ray known as fluoroscopy because the dye passes with the uterus and fallopian tubes.

The images during HSG can display problems such as an injury or abnormal structure of the uterus or fallopian tubes, or a blockage that will prevent an egg moving via a fallopian tube towards the uterus but many times the result of HSG is confusing.

In woman who wants to become pregnant sn obstruction also could prevent sperm motionless into a fallopian tube and joining which is called fertilizing an egg.

A hysterosalpingogram may also find problems on the inside of the uterus that prevent a fertilized egg from attaching implanting to the uterine wall but because it is an indirect test so it has chance of many errors.

Diagnostic laparoscopy isa very good investigation and sometime during examination of tube if hydrosalpinx or fimbrial blockage of the tube is found the it can be corrected also by a technique known as fimbriolysis or adhesiolysis.

So, in your case now the next step should be the
diagnostic laparoscopy with tubal patency test. This procedure is performed under general anesthesia, with tubal patency checked by the transcervical injection of methylene blue dye, and remains the "gold standard" for the accurate assessment of tubal patency. Visualization of the pelvic cavity by laparoscopy is necessary to ascertain tubal patency, adnexal adhesions, and ovarian mass.

Some abnormalities in female reproductive organs detected at the time of diagnostic laparoscopy can be treated during the same procedure for example, lysis of adhesions, salpingectomy, ovarian cystectomy, cauterization or vaporization of endometriotic implants.

So in our opinion you should go for a tubal patancy test by diagnostic laparoscopy.

Tnx, HSG Report.

The uterine cavity opacified and appeared elevated and inclined to the right side. The right tube opacified and appeared normal with reflex of contrast from the tube into the pelvic cavity. The left tube did not opacify.

Dear Daby

Your HSG report is normal. Many times due to spasm during HSG the dye does not show in one of tube. However, One open tube is sufficient to have child. Please consult a good infertility specialist to get your problem solved. He will also see all your hormone level as well as the follicles of the ovaries are developing correctly or not.

Patent right fallopian tube, but left tubal occludion..fundally sited uterine fibroid with submucosal component

Dear Nik
I feel above mentioned is the report of the Ultrasound scan. According to this report patient's left fallopian tube is blocked and she also has a submucous fibroid. She needs to undergo Diagnostic laparoscopy with Tubal Patency test and Hysteroscopic Myomectomy. You may consult any gynecologist for further advise.

Dear Latha,
Even if your one tube is open you can get pregnant. You should consult a gynecologist for further treatment. sometimes if the tube which has hydrosalpinx is better to remove by Laparoscopic Salpingectomy.

Uterine cavity shows normal configuration without masses or suspicious filling defects. There is filling of the left fallopian tube which appears normal, with normal free spillage into the ileum. There is no opacification of the right fallopian tube, consistent with occlusion. What does this mean can I have a baby.

Dear Lakeyshia

There is abnormal spillage in left tube and blocked right Fallopian tube. To fully diagnose your condition and to open the blocked tube you have to get diagnostic laparoscopy and tubal patency test done. This can be done even at World Laparoscopy Hospital.

The left tube is opacified in entire extent with normal caliber but no free spill is seen on left side

Dear Kiran,

my hsg report is mild hydrosalpinx then how to get pragant pls ans the qustion

Dear Suriyakalaj
It can be treated by the tubal corrective surgery (salpingostomy) by opening the tube and removing the adhesion. It can be done by laparotomy or laparoscopy procedure. After this you can try to conceive normally, if it does not work then you have to plan for IVF(In Vitro fertilisation)

Impression is mild sided hydrosalpinx with patent fallopian tube.

Dear Deepika
Please send us your complete report.
A hydrosalpinx is a distally blocked fallopian tube filled with serous or clear fluid. The blocked tube may become substantially distended giving the tube a characteristic sausage-like or retort-like shape. The condition is often bilateral and the affected tubes may reach several centimeters in diameter. The blocked tubes cause infertility.
Salpingostomy ( Tubal corrective surgery) and if adhesion is there adhesiolysis can be done for its treatment. Unfortunately, pregnancy rates tended to be low as the infection process often had permanently damaged the tubes.

Dear Divya

This is a normal hysterosalpingogram findings and thus you have good chances of conception.
Normal caliber and patent tubes are very important for natural conception and the normal endometrium lining is very important for proper implantation and growth of the baby.

Dear Suman
The report state that spillage is seen from the left side so your right tube is ok but problem is with your left side the tube is not visualised neither it shows any spillage. So it can be blocked. But as your right tube is opened you can get pregnant.

Dear Bhuwan

The condition of the tube is not good. It is totally blocked. So you need to go for the Diagnostic Laparoscopy with tubal patency test with chromopertubation test. For the further evaluation of the tube and opening of the tube.

My hsg results read :the uterine cavity is well opacified with a well rounded central lucency in keeping with air bubble. Uterine cavity Is normal in shape and out line. The mid and third right Fallopian tube is dilated and tortous. However there's forced spillage of contrast on the right. Conclusion :1. Normal uterine cavity. 2 Right hydroslapinx with forced spillage of contrast. 3 Non patent left Fallopian tube.

Reply:
Dear Maidan/Sir
Your Right tube is partially blocked with hydrosalpinx. Right fimbrioplasty need to be done. The left tube is blocked, however Lap & Dye test should be done to confirm the patency of left tube.

Hello, I am wondering how the HSG could conclude that the left is also dilated if no spillage came through at all? Please review my results and offer insights!

"Uterine cavity is normal in contour and configuration. Both fallopian tubes show dilated distal ends. Following injection of almost 20 ccs of opaque, the right tube showed intraperitoneal spill. No intraperitoneal spill of the contrast is noted on the left".

Thank you!

Dear Sir,

HSG is not a very good investigation for tubal patency. laparoscopy was found to be a better predictor of future fertility than HSG. Also in women who are at an increased risk of having subperitoneal pathology, diagnostic laparoscopy should be offered early in the infertility work-up.
