I am an endocrinologist based in Kathmandu, Nepal. Last week we diagnosed a case of Insulinoma at the head of the pancreas nearly 19 mm in size. This patient had hypoglycemia for which he was evaluated in New Delhi where he was said to have reactive hypoglycemia TWO YEARS BACK. But recent MRI shows the NET in the head of the pancreas. He also has a high calcium level with high iPTH but 24-hour urinary calcium is low, Sestamibi Scan is negative for Parathyroid adenoma. Prolactin level is normal. The patient wants to avoid Whipple surgery which seems to be too invasive. We are considering radiofrequency ablation(available in our center) or Irreversible electroporation( not available in our center) for the treatment. I came to know about your center regarding the minimally invasive enucleation technique for the Pancreatic tumor. Is this treatment justifiable? If you have experience please let me know. I hope my patient will benefit from it rather than going for invasive surgery and sequelae thereafter. Kindly let me know whom to refer this patient to in your center and if online consultation is available for my patient.
