city scan with and without contras pancreatic parenchyma appears atrophic with dilated main pancreatic duct with maximum dilatation 6.5mm in head region with 3.6mm calculus in main pancreatic duct in head region. and mild splenomegaly with long axis 12 cm give me right treatment

Dear Mr Asif
In your case of stone in main pancreatic duct extracorporeal shockwave lithotripsy can be tried. ESWL to treat a pancreatic duct calculus is a good option but it may not be successful then surgery is recommended. Progressive narrowing of the internal diameter due to inflammation of the main pancreatic duct is dangerous and it should be treated soon.
