My daughter 15 yrs diagnosed having bilateral pheochromocytoma and we want to make a laparoscopy surgical for her. Please advise whether she can have cortex sparing adrenalectomy to avoid lifetime steroid replacement for her? How much will it cost? Please reply to my email. We really need the explanation, so we can prepare everything asap from here (Indonesia)

Dear Nancy

Bilateral Laparoscopic partial adrenalectomy for pheochromocytoma is safe and technically feasible. During partial resection intraoperative ultrasonography is helpful to accurately plan resection of the tumor. If tumor location permits, the main adrenal vein should be preserved to ensure adequate vascularity for the adrenal remnant. Partial adrenalectomy has to be considered for bilateral adrenal tumors.

In a specialized center with experienced laparoscopic surgeons, laparoscopic partial adrenalectomy for pheochromocytoma is feasible even after previous open surgery on the ipsilateral adrenal gland. Adrenal-sparing surgery is indicated in hereditary syndromes such as VHL and MEN II to avoid the problems of life-long steroid replacement. Recurrences have to be expected, but further surgery may be less difficult by the previous laparoscopic approach.

MK Gupta
