Dear Sajid

Not all patients with bile duct stones can be banded
together. In essence, there are three groups.

The first is that group who, because of extreme age or infirmity are not fit for operation like laparoscopic CBD exploration. For them, endoscopic treatment is the only option.

The second group is those for whom the desirability of
removing the gallbladder is accompanied by increased risk
due to significant co-morbidity or age. It is this group for whom the decision between the two therapeutic options is unclear and further study is warranted.

The third group is those for whom there is no added risk for surgery. For them there appears to be no good reason to attempt endoscopic sphincterotomy. Considered in all respects, the evidence points to a single laparoscopic approach being the treatment of choice.

Please consult your surgeon and discuss on all these point.

With regards
