i am 40plus.Medicines and Gnrh therapy did not work on me. Hence, Doctors have suggested hysterectomy(without removal of ovaries). should i go by TAH or LAVH. Kindly tell me cost difference and also the side effects of both plus recovery time. i will be very grateful.

Dear Anu

LAVH or TLH is better option for you.

The total laparoscopic hysterectomy offers women an option that is less invasive. The whole uterus is removed by laparoscopic technique. The early stages of total laparoscopic hysterectomy are performed in the same way as LAVH. The uterine artery is secured with bipolar forceps or harmonic scalpel using intermittent small applications or by retroperitoneal uterine artery sealing using vessel sealing devices. Vagina is closed laproscopically in TLH, using suture material and laparoscopic needle holder.

If the uterus is very large, it is removed by morcellation. In cases of previous scar, severe endometriosis, pelvic inflammatory disease, pelvic tuberculosis, there may be presence of adhesions. Ovariectomy can be performed routinely during hysterectomy, in peri- menopausal women or any other benign tumour of ovary.

Advantage of TLH-

Incisions are VERY small.

Scars, pain, and recovery time are significantly less

Minimal post-surgical pain medication use.

It is physically less traumatic than a routine hysterectomy.

Early discharge faster recovery

NO vaginal shortening.
