my DTPA Diuretic Renogram Study shows functional parameters are Left Kidney - 4.21% (percentage function), 2.97 ml/mins(GFR) In impression it says - poor glomerular and cortical function of the grossly hydronephrotic left kidney with significant cortical thinning and tight PUJ obstruction on the left side, contributing to 2.97 ml/mins to the total GFR |

Dear Ravi Kumar The obstruction of PUJ needs to be removed so that urine can pass freely from the kidney down to the bladder. This surgery can be accomplished by several means: 1, By cutting out the PUJ obstruction and joining the kidney onto the ureter called pyeloplasty. Pyeloplasty i.e. cutting out the obstruction has the best results and lasts for the longest period. This can be achieved through a traditional surgery either by open pyeloplasty or by keyhole surgery laparoscopic pyeloplasty. Making a cut in the PUJ obstruction so that it splits open and becomes wider that way called as endopyelotomy which is less effective than cutting it out altogether, but is possibly better than bursting it with a balloon. In some situations, it is dangerous because of neighbouring blood vessels. Bursting the obstruction with a balloon. Bursting the obstruction with a balloon is quick, the least invasive but is less effective and lasts for the shortest period. Furthermore, it produces scarring that can make corrective surgery more difficult. For some patients it is the best option because poor health makes other treatment dangerous. So in our opinion you should consult some good urologist for further investigation and evaluation so to decide which surgery will be better for you. With regards M.K. Gupta |

Dear Doc, My son has diagnosis in march 13 in his health check up that he have only one kidney by birth in good size, Now he is 22 years old, on diagnosis he has 3.8 serum creatinine, Now he have 4.9 serum creatinine,He has on medication from march 13. Please suggest me , Is there any treatment. Dear Mr. Sharma At this stage your Son need medical management but in future he may require kidney transplant. Please consult a good nephrologist.
