Dear Irene Paraesophageal hiatus hernia (PEHH) is an uncommon type of diaphragmatic hernia in the pediatric age group. Preoperative assessment included chest x-ray and CT scan. We suggest that laparoscopic repair of PEHH in infants is safe and preferred mode of the treatment. The hernia involves the stomach, spleen, and splenic flexure of the colon, which have herniated into the left hemithorax, causing compression of the left lung base and slight displacement of the mediastinal structures towards the right. This condition appears to have been present since birth, but we have only recently become aware of it. Fortunately, my wife is currently not experiencing any significant symptoms or discomfort. We would like to seek your advice regarding the appropriate course of action in this case. Would you recommend proceeding with surgical repair, or would it be reasonable to monitor the condition without surgery since she is currently asymptomatic?
