Dear Aamani

If Medical treatment is not helping you in Polycystic disease of ovary you should opt for laparoscopic ovarian drilling. Ovarian drilling is really a surgical procedure that is used to help patients with polycystic ovarian syndrome (PCO) achieve pregnancy. Ovarian drilling, done during laparoscopy, is really a procedure in which a laser fibre or electrosurgical needle punctures the ovary 4 to Ten times. This treatment produces a dramatic cut in male hormones within days and is often performed in females who have polycystic ovary syndrome (PCOS). Research indicates that as much as 80 percent of patients will benefit from such treatment. A lot of women who fail to ovulate with clomiphene or Metformin therapy will respond when these medications are reintroduced to the system after ovarian drilling. Side effects are rare, but may result in adhesion formation or ovarian failure if there are complications throughout the procedure.

Some reproductive endocrinologists have expressed mixed feelings about this technique. Some think that any take advantage of ovarian drilling can be obtained non-surgically using glycophage.

We still find it a good technique for various kinds patients. Included in this are younger patients who neglect to respond to some version of clomiphene citrate therapy and who do not wish or can't afford IVF. They aren't candidates for gonadotropin therapy with IUI simply because they must great a danger of multiple gestations. We like to use ovarian drilling in those patients who cannot tolerate glucophage. I feel that ovarian drilling functions by establishing small areas of fibrosis in the ovary. Oocytes (eggs) which are located near these areas of fibrosis can be cultivated follicles inside a more normal environment endocrinologically that existed before the ovarian dilling. Such oocytes may be better quality eggs when they ovulate.

One of the leading concerns with ovarian drilling is adhesion formation following the procedure. I haven't seen significant adhesion formation on any second look laparoscopies that I did in patients who had ovarian drilling. I continue to believe that this is due to my utilization of large volumes of fluid within the abdomen. I've done two second look laparoscopies in patients who had ovarian drilling made by other doctors with presumably other techniques in which the pelvic organs had severe adhesion formation. For all medical procedures, it is desirable to become treated by somebody who has much experience as possible with that procedure.

Yes, there's hope, but it's going to take you being a lot more proactive than triyng for 2 years. First, you need to get tested. You need to know what the enemy is, so you can face it head on. Some tests you'll need: CD3 blood work to include FSH, E2, LH a CD3 antral follicle count .an HSG to see if your tubes are open a progesterone test post-ovulation to see if your levels are good (done at 7 days past ovulation). These basic tests will give you a pretty good picture of where you stand fertility wise. Second, your partner needs a sperm analysis. Even if he's had kids before. There's such a thing as secondary infertility. Don't assume the problem is 100% you.And yes, at your age you NEED an RE. REs are highly trained in this area and you need someone with that kind of expertise. If you had seen an RE 2 years ago you probably would have been pregnant already. An RE knows far more about fertility than an OBGYN and will know exactly what tests you need. You may have to interview a couple of them before finding one that's AMA (advanced maternal age) friendly. It could be that you just need some extra progesterone support (common in age 40+ women) after ovulation. Do you chart your temps? Charting will help you to become familiar with your cycle. I highly recommend you start. FertilityFriend.com has a great website for this. The problem may be an easy fix, it may be more complicated. But you won't know which until you see the RIGHT doctor and have the RIGHT tests done.Good luck.

Sir,I have bi-lateral polycystic ovaries.I don't get my mens on my date and the quantity of bleeding is also very less,so what can I do?

You should get medical management first with Clomiphene therapy. It is typically used for 5 consecutive days early in the menstrual cycle, for 3 to 6 monthly cycles. It may take several cycles to find the right dose to stimulate ovulation. After that dose is determined, a woman will take the drug for at least 3 more cycles. If you does not become pregnant after 6 cycles, it is unlikely that further clomiphene treatment will be successful and in this case laparoscopic ovarian drilling should be thought.
