Institute: WLHThe repair of injury or end to end anastmosis of small diameter structures like fallopian tube,ureter or common bile duct etc. can only be done with hand sewn repair or anastmosis. The principals of anastmosis as observed in open surgery remain the same viz. 1.The posterior layer must be taken first, 2. The knot should be tied outside the Lumen and 3 .Single layer interrupted sutures are better than the continuous sutures. Dr R K Mishra today demonstrated the art of laparoscopic anastmosis and thereafter doctors had hands on training and they realised the intricacies of the procedure.
Dissection in laparoscopic surgery also poses a stiff challenge to the surgeons. The adhesiolysis here is done with the help of energy souces or with sharp micro scissors. Adhesions with bowel are dissected using sharp scissors and energy source is not used. Safe use of Radiofrequency Monopolar and Bipolar intruments is of utmost importance in laparoscopic surgery. Today Dr J S Chowhan demostrated the use of scissors for sharp dissection and the doctors performed the laparoscopic dissection of the Gall bladder/Ovarian cyst without rupture of the organ.
