This video demonstrates Laparoscopic Hernia Repair Large Sac TAPP Repair. It was an irreducible hernia with 2 meters of intestinal content in the sac. Ger has described the first potential laparoscopic inguinal hernia repair in 1982. He describes a metallic clip applying device to close the hernia sac during laparotomy for other operations. He eventually describes one case of laparoscopic inguinal hernia repair in a similar fashion with metallic lips only. His approach was applicable to hernia sacs with defects less than 1.25 cm. He did not describe reconstructing the inguinal floor and his approach was not applicable for direct inguinal hernias. As with the transabdominal approach (TAPP), the principles touted by Rives and Stoppa for the open preperitoneal repair of a large mesh providing coverage overall defects, distributing intra-abdominal pressure over the large mesh area, and requiring minimal fixation, were primary principles of the laparoscopic approach to inguinal hernia repair. 

Hernia is a common condition where an organ or tissue protrudes through a weak spot in the abdominal wall. Among the various surgical techniques available today, laparoscopic hernia repair has emerged as a preferred choice due to its minimally invasive approach, faster recovery, and reduced postoperative pain. For patients with a large hernial sac, the Transabdominal Preperitoneal (TAPP) repair technique is especially effective.

Understanding Large Sac Hernias

A large sac hernia occurs when the protruding peritoneal sac is significantly sized, often containing intestines or omentum. These hernias are more challenging to repair because of the increased risk of complications such as bowel obstruction, incarceration, or recurrence if not handled carefully. Traditional open surgery can be effective but often results in longer recovery times and larger scars.

What is TAPP Repair?

TAPP (Transabdominal Preperitoneal) Repair is a laparoscopic technique in which surgeons access the hernia from within the abdominal cavity. Unlike open surgery, TAPP repair allows the surgeon to place a mesh over the defect without making large incisions. The steps include:

Access and Visualization: Small keyhole incisions are made in the abdomen, and a laparoscope is inserted to visualize the hernia.

Dissection of the Hernial Sac: The hernial sac is carefully dissected from surrounding tissues. In the case of a large sac, meticulous dissection is crucial to avoid injury to intestines or vessels.

Placement of Mesh: A synthetic mesh is placed over the hernia defect in the preperitoneal space to reinforce the abdominal wall.

Peritoneal Closure: The peritoneum is sutured or tacked back in place to cover the mesh, preventing adhesion to intra-abdominal organs.

Advantages of Laparoscopic TAPP for Large Sac Hernias

Minimally Invasive: Smaller incisions lead to less postoperative pain and quicker recovery.

Better Visualization: The laparoscope allows precise identification and repair of the hernia and surrounding structures.

Lower Recurrence Rates: Proper mesh placement reduces the risk of hernia recurrence.

Early Return to Normal Activities: Most patients resume daily activities within a few days to a week.

Considerations and Challenges

Repairing a large sac hernia laparoscopically requires advanced surgical skill. Challenges include:

Risk of injury to abdominal organs during dissection.

Difficulty in reducing large sacs without causing tension.

Ensuring proper mesh coverage to prevent recurrence.
