Total Laparoscopic Hysterectomy by Two 5 mm Ports - Single Instrument Surgery
https://www.laparoscopyhospital.com
This video demonstrates Total Laparoscopic Hysterectomy by Two Ports. Only Two 5mm ports were used in this surgery. One telescopic port and only one working instrument are used. This is the first single-handed Total laparoscopic hysterectomy done by anyone before. Many surgeons are performing SILS hysterectomy. In single incision laparoscopic surgery one large 20mm-30mm port is used and in a single port three instruments are introduced. In this two-part surgery, only one operating instrument is used. Single-incision laparoscopic surgery is an alternative to conventional multiport laparoscopy. Single-access laparoscopy using a transumbilical 20 mm port does not give cosmetic benefits because the surgical incision is large in the umbilicus and increases hernia infection and morbidity of minimally invasive surgery. With two 5 mm ports, there is no chance of hernia excellent cosmetic benefits and minimum pain is observed. The patient is discharged only a few hours after surgery. This is a single-handed surgery so surgeons need good suturing skills.
Discover the minimally invasive approach to Total Laparoscopic Hysterectomy (TLH) using just two 5 mm ports and a single instrument technique. In this video, we demonstrate a highly advanced, patient-friendly procedure designed to reduce postoperative pain, minimize scarring, and speed up recovery.
Highlights of this surgery:
Use of only two 5 mm laparoscopic ports
Innovative single instrument technique
Reduced operative trauma and faster healing
Step-by-step demonstration of surgical precision
This procedure is ideal for gynecological surgeons aiming to adopt less invasive, ultra-modern techniques in laparoscopic surgery.
About the Surgeon:
Performed by Dr. R.K. Mishra, a pioneer in minimally invasive gynecologic and laparoscopic surgeries at the World Laparoscopy Hospital, India.
Minimally invasive surgery has revolutionized gynecological procedures, offering patients faster recovery, less pain, and minimal scarring. Among these advances, Total Laparoscopic Hysterectomy (TLH) performed using two 5 mm ports with a single instrument technique represents a significant step forward in patient-friendly surgery.
What is Total Laparoscopic Hysterectomy (TLH)?
TLH is a surgical procedure in which the uterus is removed entirely using laparoscopic techniques. Unlike traditional open surgery, TLH is performed through small incisions, allowing the surgeon to operate with a camera and specialized instruments, ensuring precision and minimal trauma to surrounding tissues.
The Two-Port, Single Instrument Approach
Traditionally, TLH requires multiple ports (3–5) for instruments and visualization. The two-port method utilizes:
A 5 mm laparoscopic port for the camera.
Another 5 mm port for a single operative instrument.
This technique leverages advanced surgical skills and specialized instruments to perform all necessary steps of the hysterectomy, including:
Detaching the uterus from supporting ligaments
Controlling blood vessels
Removing the uterus safely through the vagina or a small incision
Advantages of This Technique
The two-port, single instrument TLH offers several key benefits for patients:
Minimal scarring – Only two small 5 mm incisions are needed
Reduced postoperative pain – Less tissue trauma leads to quicker recovery
Faster recovery – Patients can resume daily activities sooner than conventional surgery
Lower risk of complications – Smaller incisions reduce the chance of infection and bleeding
Who Can Benefit?
This technique is suitable for patients requiring hysterectomy for:
Fibroids
Adenomyosis
Abnormal uterine bleeding
Early-stage uterine pathology
However, careful preoperative assessment is necessary to ensure patient safety and procedural feasibility.
Conclusion
The two 5 mm port, single instrument approach to TLH demonstrates the evolution of minimally invasive gynecologic surgery. It combines surgical precision, patient comfort, and aesthetic outcomes, making it a preferred option for both surgeons and patients.
Performed by expert laparoscopic surgeons such as Dr. R.K. Mishra at World Laparoscopy Hospital, India, this technique represents the forefront of innovation in gynecological surgery.

Single-incision laparoscopic surgery is an alternative to conventional multiport laparoscopy. Single-access laparoscopy using a transumbilical port affords maximum cosmetic benefits because the surgical incision is hidden in the umbilicus. The advantages of single-access laparoscopic surgery may include less bleeding, infection, and hernia formation and better cosmetic outcome and less pain. The disadvantages and limitations include longer surgery time, difficulty in learning the technique, and the need for specialized instrument

TLH by SILS with intracorporeal suturing and knot tying is feasible and can be performed by surgeons worldwide with the use of a low-cost single-port device and conventional, reusable laparoscopic instruments. Less postoperative pain and better cosmesis seem to be an advantage of SILS; however, larger cohort studies are necessary to encourage or discourage this minimally invasive procedure.
