Laparoscopic Appendectomy in Pediatric Patient
Gen Laparoscopic Surgery / Sep 9th, 2021 10:10 am     A+ | a-

This video demonstrates Laparoscopic Appendectomy in Pediatric Patient perform by Dr. R.K. Mishra at World Laparoscopy Hospital. Laparoscopic appendectomy is safe and effective in treating children with suspected appendicitis. It may be safely performed as fast-track or same-day surgery, in select children without perforated appendicitis, with a postoperative stay of ≥24 hours in the majority of such patients. The recurrence of appendicitis caused by an infection -re the usual reasons for removal. The laparoscopic technique seems to be safer than open appendectomy for acute appendicitis in children. The laparoscopic approach is most frequently used, except for young children. Superficial surgical site infections are more frequent after open surgery only in patients with complicated appendicitis. The normal appendix rate is low, most likely because of routine preoperative imaging.

Appendicitis is one of the most common surgical emergencies in children. With advances in minimally invasive surgery, laparoscopic appendectomy has become the preferred treatment for acute appendicitis in pediatric patients. Compared to open surgery, laparoscopy offers better visualization, reduced postoperative pain, faster recovery, and superior cosmetic outcomes, making it especially suitable for children.

Indications for Laparoscopic Appendectomy

Laparoscopic appendectomy is indicated in most pediatric cases of:

Acute uncomplicated appendicitis

Perforated appendicitis (in experienced centers)

Recurrent appendicitis

Diagnostic uncertainty in abdominal pain

Contraindications are rare but may include severe cardiopulmonary instability or extensive abdominal adhesions.

Preoperative Evaluation

A thorough clinical assessment is essential and includes:

Detailed history and physical examination

Laboratory tests (CBC, CRP)

Imaging such as ultrasound or CT scan (if required)

Preoperative antibiotics to reduce infection risk

Parental counseling and informed consent are crucial components of pediatric surgical care.

Surgical Technique

Laparoscopic appendectomy is typically performed under general anesthesia using a three-port technique:

A 5–10 mm umbilical port for the laparoscope

Two additional working ports in the lower abdomen

The appendix is identified, the mesoappendix is divided using energy devices, and the base of the appendix is ligated with endoloops or staplers. The appendix is then removed through a port site. The abdominal cavity is irrigated if contamination is present.

Advantages in Pediatric Patients

Laparoscopic appendectomy offers several benefits over open surgery:

Reduced postoperative pain

Shorter hospital stay

Faster return to normal activities and school

Lower wound infection rates

Better cosmetic results

Improved visualization, especially in obese children

These advantages significantly enhance recovery and overall patient satisfaction.

Postoperative Care

Postoperative management includes:

Early mobilization

Gradual resumption of oral feeding

Pain control with minimal analgesics

Monitoring for complications such as fever or wound infection

Most children can be discharged within 24–48 hours in uncomplicated cases.

Complications

Although laparoscopic appendectomy is safe, potential complications include:

Surgical site infection

Intra-abdominal abscess (especially in perforated cases)

Bleeding

Port-site hernia (rare)

Early diagnosis and prompt management help ensure excellent outcomes.

Outcomes and Prognosis

Numerous studies have demonstrated that laparoscopic appendectomy in children is associated with excellent clinical outcomes and low complication rates when performed by trained surgeons. The minimally invasive approach has become the gold standard for pediatric appendectomy worldwide.

Conclusion

Laparoscopic appendectomy is a safe, effective, and preferred surgical approach for treating appendicitis in pediatric patients. With its multiple advantages and rapid recovery, it represents a significant advancement in pediatric minimally invasive surgery and continues to improve the quality of surgical care for children.
