Advantage of Total Laparoscopic Hysterectomy
General Surgery / May 20th, 2017 2:54 pm     A+ | a-

The procedure of removing the uterus of a woman is called Hysterectomy. There are different techniques to perform it. Some of the first used procedures required at least 6 weeks of recovery and had several complications.

Fortunately, science has advanced to an extent in which it is easier to perform a hysterectomy through a procedure called total laparoscopic hysterectomy. This is not as invasive as other surgeries. Here is a comprehensive research on the advantages this procedure has over other alternatives.

Why should you need to perform a Hysterectomy?

The procedure of removing the uterus is usually associated with the eradication of cancer on the ovaries, cervix, and uterus. By eliminating the source of cancer, it is easier to control it and prevent it spreads to other organs. But there are other aims to perform such procedure. Some of them are:

·        Adenomyosis.

·        Endometriosis.

·        Excessive vaginal bleeding.

·        Pelvic pain (when it is chronical).

·        Uterine fibroids.

·        Uterine prolapse.

All of these are usually a source of pain and bleeding. When you experience any of these issues, a hysterectomy is the last option. There are other treatments to cure these diseases and ease the pain and discomforts. Only when the risks of leaving the uterus where it is are less than the risks of taking it out, the gynecologist will opt for removal.

Types of Hysterectomy

Depending on which part of the female reproductive system is removed, the hysterectomy is named differently. Here are the options:

·        Supracervical or Subtotal Hysterectomy. The cervix is preserved, and just the upper side of the uterus is removed.

·        Total hysterectomy. The cervix and uterus are removed.

·        Radical Hysterectomy. The cervix, uterus and some part of the vagina are removed.

·        Oophorectomy. The ovaries are removed.

·        Salpingectomy. The tubes connecting the ovaries are removed.

·        Hysterectomy and Bilateral Salpingectomy-Oophorectomy. The uterus, ovaries and connecting tubes are removed.

Depending on the reason for removal, a different procedure is chosen. The most radical options removing everything are only used when there is cancer spread all over the area. Whenever the hysterectomy procedure is done with the assistance of a camera using small cuts, then it is called a laparoscopic hysterectomy.

Types of Laparoscopic Hysterectomy

There are several kinds of laparoscopic hysterectomy, depending on the procedure and specific tools to assist the surgeon. New techniques are being developed, and the following are just the most common classifications of laparoscopic hysterectomies:

·        Traditional Laparoscopic hysterectomy. The camera and other tools are inserted through various cuts on the belly area.

·        Single Site Laparoscopic Procedure. This requires a single cut. The belly button is used as an entrance, and all tools and camera go in through a single tube.

·        Laparoscopic-assisted vaginal hysterectomy. The procedure is done as explained previously, except that the uterus removal is done through the vagina.

·        Laparoscopic hysterectomy with robotic assistance. Instead of manipulating the tools directly, the surgeon makes use of a tiny robot. It responds to the natural movements of the wrist, and it can be assisted by a 3D image.

All the previous procedures require the connection of an LCD screen to get a view of the inside. The surgeon manipulates the tools without actually getting inside the body. From the outside, he gets to know what is happening inside thanks to the HD camera that is inside the body.

Abdominal Hysterectomy

Before we get to the advantages, let’s explain a few things about an abdominal hysterectomy. This procedure is very invasive and requires a major incision to open the abdominal area. That way, the surgeon can have access to the vagina and other genital organs to remove them.

It usually needs at least 3 days in the hospital, plus a week of full rest recovery. Then, before complete recovery, a period of 6 weeks is necessary. Afterward, there can be complications such as pain and bleeding due to the significant intervention.

Advantages of Laparoscopic Hysterectomy over the regular Abdominal HysterectomyJust by having a less invasive procedure, the benefits of laparoscopic hysterectomy over the traditional abdominal hysterectomy seem obvious. These are the most important remarks:

·        A laparoscopic procedure requires incisions of only 1 centimeter, which has the following advantages:

o   Less pain after the surgery.

o   Fast recovery.

o   Fewer if no scars.

·        The laparoscopic surgery needs only one night at the hospital versus 3 to 5 days for a regular abdominal hysterectomy.

·        There are few complications like:

o   The risk of infections.

o   Bleeding.

o   Other abnormal formations.

·        There are few complications in the urinary tract.

·        The sex drive is recovered faster.

In summary, there are better chances to go back to normal faster. Depending on the procedure, the cervix is preserved or not. A total laparoscopic hysterectomy takes it out too. Some doctors think this is an advantage because it eradicates the chances of having cervical cancer.
