Laparoscopic surgery is widely recognized as one of the most important advancements in modern surgery. The minimally invasive approach can provide several benefits, including smaller wounds, less postoperative discomfort, faster mobilization, and quicker recovery. Despite these advantages, laparoscopic surgery must always be selected according to the individual patient's medical condition. Severe COPD and significant cardiac disease can increase the physiological challenges associated with laparoscopy.

The main reason is the creation of pneumoperitoneum. During laparoscopic surgery, carbon dioxide gas is used to inflate the abdomen and create sufficient operating space. Patients with severe COPD may have difficulty tolerating increased carbon dioxide absorption and changes in respiratory mechanics. Patients with severe heart failure, unstable cardiac conditions, significant ischemic heart disease, or other serious cardiovascular problems may therefore face increased perioperative risk.

For this reason, the decision to perform laparoscopic surgery should never be based only on the benefits of minimally invasive surgery. A complete evaluation involving the surgeon, anesthesiologist, and when necessary, cardiologist or pulmonologist is important. Investigations such as cardiac assessment, pulmonary function evaluation, and optimization of existing medical conditions may be required.
