VENTRAL HERNIA REPAIR WITH SEPSIS

I repaired an incisional EPIGASTRIC HERNIA with a dual mesh a few days ago. Insufflation was done though the smiling incision. Now the patient has developed an abdominal wall abscess around the umbilical wall area. The abscess is in the wall of the abdomen not intra-peritoneal. I have drained the abscess. Should I consider taking out the mesh or rather wait and see?

Dear Dr Chamisa

Ideally in incisional hernia the umbilicus should not be disturbed or any skin where mesh is there should not be breached. Ideally the access should be from palmer's point. Now in this situation please give injectable high antibiotic for 7 days preferable cefoperazone. If it works it is OK otherwise removal of the mesh should be considered if the abscess does not heal within 6 week.
