Dear Ali

A varicocele forms when valves inside the veins along the spermatic cord prevent blood from flowing properly. This causes the blood to back up, leading to swelling and widening of the veins.

Varicoceles usually develop slowly. They are more common in men ages 15 - 25 and are most often seen on the left side of the scrotum. Varicoceles are often the cause of infertility in men.

A scrotal support or snug underwear may provide some relief of the pain or discomfort. If pain continues or other symptoms occur, further treatment may be needed.

Surgery to correct a varicocele is called a varicocelectomy. It can be performed by laparoscopy. The urologist will make a cut, usually in the lower abdomen, and tie off the abnormal vein. Blood will now flow around the area into normal veins. Keep an ice pack on the area for the first 24 hours after surgery to reduce swelling.

Laparoscopic varicocele ligation is a simple and safe technique, causing minimal morbidity and enabling a rapid return to normal activity. The success rate can be improved by accurate identification of the testicular vessels with the use of a laparoscopic Doppler probe.

hi sir this is amar from india and my age is 21.surgery is attempted for testicular torsion to my right testicle and after the operation i got a report that no fluid in both testicle but grade-1 varicocele at left testis. do i have any natural treatment or exercise for this. or should i go for surgery? i feel a little comfort due to this varicocele. can reply to mail :
amardeep.chintu@yahoo.com

Dear Amardeep

There is an association between varicoceles and infertility. The incidence of varicocele increases to 30 percent in infertile couples. Decreased sperm fertility, decreased motility of sperm, and an boost in the amount of deformed sperm matched to varicoceles. Some experts believe that blocked and enlarged veins round the testes, called varicoceles, cause infertility by raising the temperature within the scrotum and decreasing sperm production. Testicular Atrophy - Shrinking from the testicles is the one other manifestation of varicoceles. Often, as soon as the testicle is repaired it will resume normal size.

Laparoscopic surgery is also a good treatment modality of varicocele. As soon as the patient receives anaesthesia, a cut is done from the skin over the scrotum, scaling down for the testicular veins, and tying them served by sutures. Although patients leave a medical facility within a day, there is a two to three-week recovery period.

Hi
I had grade 1 varicocele in my left testicle. Six months ago a doctor
in Iran, treating my varicocele, had performed varicocele repair open
surgery. Since the surgery I had a terrible pain in my left testicle
and my left testicle has been placed upper from the initial position
and now, after six months, I always have pain and feel like pull or
torsion on my left testicle. Also the cut area is painful and I can't
touch it. This suffers me very much and I can't do my routine
functions easily. Please tell me why this pain has arisen? Will this
pain last for ever? What's the problem?
Thanks

Dear Babak,

You have to consult the doctor who has performed the surgery and get some investigation done. It may be port side infection it should be checked for atypical micro-bacteria also.

Dr I have grade one varicocel on both side. Can I do my regular office job . if I do 8 hours job.that job affect on my varicocel or not.what can I do . I am really scared about my varicocele.

Dear Vijay

If you will do simple office work grade one varicocele will not have any problem with that. Often, varicoceles are not treated. Treatment is offered for males who have:

fertility problems and pain in testes. The left testicle growing more slowly than the right. Boys with a smaller left testicle are thought to have a higher risk for fertility problems when they get older.

There are no drugs to treat or prevent varicoceles. But pain killers may help with pain.

When needed, surgery is the main form of treatment. Embolization is a non-surgical treatment option.

After ultrasound dictated Grade 1 Varicocele . My testicle size is normal. but i feeling slow pain in my scrotum. Medicine treatment is not available So What can i do pl suggest.

Dear Mr Joshi

It’s not always necessary to treat a varicocele. However, you may want to consider treatment if:

your varicocele causes pain
your varicocele causes testicular atrophy
your varicocele causes infertility

you’re thinking about assisted reproductive techniques
This condition can cause problems with testicular functioning in some people. The earlier you start treatment, the better your chances of improving sperm production.

Wearing tight underwear or a jock strap can sometimes provide you with support that alleviates pain or discomfort. Additional treatment such as laparoscopic varicocelectomy and varicocele embolization might be necessary if your symptom persist.

Mere ultrasound mai hyderocele Normal Par grade 1 varicocele hai mujhe hyderocele mai humesa halka halka sa pain hota hai mai kya karu plz sir help me mujhe kyon sa treatment Karwna chahiye aur ye mere Right side mai pain hota hai.

Dear Deepak

For grade I varicocele generally treatment is not required if it is asymptomatic. The goal of varicocele treatment is to stop the backward flow of blood from the body to the scrotum, and therefore to cool off the testicles. To achieve this, veins leading from the testis to the body are tied off or interrupted as completely as possible. A single vein is left open ie. vasal vein that is not subject to the same issues as the varicose veins and allows the blood to leave the testicle after surgery. There are several ways to treat varicoceles. Surgical or incisional methods are performed in the upper scrotum which is called subinguinal, groin area inguinal or lower abdomen retroperitoneal.

The procedure can also be performed with key hole surgery (laparoscopy) and also by an interventional radiologist through a procedure called venography and embolization.

Please contact a urologist to suggest you if surgery is required. Alternatively you may come to World Laparoscopy Hospital and after examination we can give you proper advice.

I have varicocele grade 1 on the right side testicle does it is repaired without surgery and how much time to required its repaired to normal.

Reply:

Dear Sir

Most of the varicocele does not create any problem so surgery is not necessary. As most of the patients do fine with conservative management.
