I am 28 year old male. I have a history of Esophagitis, Gastritis Antral and Erosive Duodenitis and was on Pantocid 40mg/Rabicip 20mg for 12 years continuously. I had all these health problems due bad habit of smoking in a very young age when i was 16-17 years old. I got this bad habit in a very young age. I used to smoke in empty stomach. I smoked cigarettes for 6-7 years. When i got very sick and developed lots of gastrointestinal problem which was diagnosed when i was admitted in Ruby Hall Clinic in Pune in year 2000, i quit smoking in year 2002. Since then my medicines continued. I consulted Gastroenterologist in Chennai and he advised endoscopy and liver function test. Endoscopy report showed (OESOPHAGUS - Mucaza normal Lax OGJN GMP+ gr. A Reflux Small H.H) Diagnosis: Reflux Esophagitis and Small Hiatus Hernia. You also told me that my stomach was distended. You then prescribed me Nexpro 40mg b.d., Ciza 10mg b.d. 1/2 hour before meal and Diovol Gel 2 tablespoon 1 hour after meals for 2 months. He also advised me to take Glutamine Powder if it helps. He told me to not to lie down after eating meals for 2 hours. He further said i should avoid oil in the diet completely and if i don't find relief, i should consult him after 1 week. By looking at my diagnosis, do i require Nissen Fundoplication surgery? Do small hiatus hernia require surgery? Its been 3 years since my initial diagnosis and due to painful eating and eating only bland food, i have already lost more than 18 kgs of body weight. Will i develop Barrett's Esophagus or esophageal cancer if i avoid surgery? I am going to see my Gastroenterologist in November this year. I am really scared. Any suggestions will be helpful.

Dear Ayush

A hiatal hernia often is caused by weak muscles and tissue within and around the hiatus.

If you often have symptoms or they are severe, you may have gastroesophageal reflux disease (GERD). A hiatal hernia can lead to GERD, and people often have both conditions at the same time.

For those who have mild symptoms, We will suggest you lifestyle changes as well as perhaps nonprescription medicines. Here are a few circumstances to try:

Alter your eating habits.
It is advisable to eat several small meals rather than 2-3 large meals.

When you eat, wait Two or three hours before you decide to take a nap. Late-night snacks are not advisable.
Chocolate, mint, and alcohol will make GERD worse. They relax the valve between your esophagus along with the stomach.

Spicy foods, foods that have a great deal of acid (like tomatoes and oranges), and low may make GERD symptoms worse in a few people. If your symptoms are worse when you consume a particular food, you might stop eating that food to find out if your symptoms get better.
Tend not to smoke or chew tobacco.

When you get heartburn at night, boost the head of the bed 6 in. (15 cm) to 8 in. (20 cm) by putting the frame on blocks or placing foam wedge beneath the head of the mattress.

Usually do not wear tight clothing around your middle.
Shed weight if you want to. Losing just Five to ten pounds might help.

In the event you will have symptoms and have severe symptoms, you might have GERD. Lifestyle changes can help, as well as you should get advice of a physician who may prescribe medicine. In severe cases, fundoplication surgery can be used to pull the hernia into the belly.

Thank you Dr J S Chowhan for the reply. I have tried everything what you have said for the last 3 years. I have also purchased a hospital bed for just to raise it and sleep in it. But i have not found any relief in the intensity of the stomach pain. Gasteroenterologists here in my home town have prescribed me HP kit which is a combination of amoxicillin, tinidazole and omeprazole to treat h. pylori infection. What should i do? I am not able to eat properly due to lots of stomach pain and have reduced more than 25 kgs of body weight. I have become very thin and i don't want to loose any more body weight due to painful eating. I am very scared. What should i do? I cannot come over to Delhi to see you because i don't have any relatives there. I am going in october this year to chennai to see Dr Selvakumar in Selvarangam Hospital Anna Nagar Chennai. I can go to chennai because my uncle is well settled there. Do you know any other good surgical gastroenterologist in chennai whom i can consult to? Other than him i only know of Dr Radhakrishna Patta and Dr Rajkumar Palaniappan from Apollo Hospitals Chennai. Sir any advice from you will be very helpful. Waiting for your reply.

Dear Ayush

These are the good doctors and hopefully your problem will be solved.

My father has a gastro problem from last 2 years. He is suffering from bloating, stomach pain and loosing weight. He has done endoscopy in last week. Reports are showing that small flat antral mass with normal mucosa and umbilication. My doctors said to me that reports are normal. Then why my father is suffering for bloating, pain etc. Please suggest...

Dear Mr Jayanta

If endoscopy is normal further evaluation is required for your father. You should consult a physician to do some blood test like: Blood Sugar, LFT, KFT, TB Gold to rule out tuberculosis and Thyroid Profile complete hemogram etc..

The GIT problems many time related to systematic diseases and proper diagnosis is required. Consultation with a gastroenterologist is required then only proper advice is possible. Alternatively you can bring your father to our hospital.
