Swelling, burning, and persistent pain in the testicles, especially after ongoing treatment for 3 months without improvement, is a serious concern that needs further evaluation. Since you also had testicular surgery in childhood, there may be complications like nerve damage, scar tissue formation, or issues related to blood flow (such as varicocele or epididymitis).

According to doctors, if symptoms continue despite treatment, the following steps should be taken:

Ultrasound with Doppler Study: A high-resolution scrotal ultrasound can help detect infection, fluid collection (hydrocele), varicocele, tumor, torsion, or scarring. Doppler helps check blood flow to the testicle.

Urological Evaluation: A specialist might need to evaluate for chronic epididymitis, post-surgical nerve entrapment, or even a referred pain from kidney or prostate issues. Sometimes, the pain is due to previous surgery-related nerve hypersensitivity.

Urine and Semen Tests: To check for persistent infections or inflammation. A se-xually transmitted infection (STI) or chronic prostatitis can also cause such symptoms.

MRI Pelvis or Lower Abdomen (if needed): If ultrasound is inconclusive, further imaging may help.

Treatment Options: Depending on the diagnosis, doctors may recommend:

Antibiotics or anti-inflammatory medication for infection or inflammation.

Nerve pain medications like pregabalin or amitriptyline if nerve damage is suspected.

Surgical intervention (in rare cases) if scar tissue or varicocele is the cause.

Home Care:

Wear supportive underwear.

Apply cold packs to reduce swelling.

Avoid heavy lifting or excessive movement.
