Advancement in technology cann't replace the skill acquisition was learnt by the surgeons at the beginning of the Laparoscopic Surgery era. In a training center in Holland the master trainers observed that even senior surgeons would take 8 to 10 minutes in tying an intracorporeal surgeons knot but this reduced to 2 minutes after 2 weeks training. Laparoscopic Surgery is ot a surgery in reality rather it is a surgery on image. And the minimal access surgeon only has two dimensional vision of the image. He can see normally only length and breadth. Third dimension i.e. depth is not available, rather some depth cues has to be learnt by him to do the proper surgery. In Laparoscopic Surgery hand eye coordination is of utmost importance. Moreover no assistant hand come for any help. With one hand the surgeon operates and his other hand works as assistant. He has to develop ambidexterity and both his hands should work as surgeon and assistant hand. The use of laparoscopic minimal access surgical procedures without previous laparoscopic training has grown rapidly. At the same time, there have been allegations of increased complications among less experienced surgeons who has attended one or two days fractured courses. While laparoscopic surgeons appear to learn this laparoscopic cholecystectomy procedure rapidly in wet lab but live demonstration and assisting in operation theater has its own value, institutions might consider requiring minimal access surgeons to move beyond the initial learning curve before awarding Fellowship in Minimal Access Surgery. Advances in minimally invasive surgical techniques will need new types of instrument and functionality of those instruments are important for surgeons and gynecologists to learn. Nowadays many new instrument is invented which include a new class of multifunctional instruments capable of performing greater than 1 task with a single group of working jaws.
