American Surgeons are now appreciating and using the Indian techniques to dicrease the cost of laparoscopic surgery. Endoscopic surgery is a rapidly defusing treatment approach susceptible to continual technological advance. It offers great possibility of economic benefits in addition to health gains, but as shown in the article within this issue by Kesteloot and Penninckx, the relative price of open versus endoscopic surgery is a quite complex issue. The main factors influencing the costs and effectiveness of endoscopic surgery are reviewed and also the requirement for more and better evaluation studies highlighted.

Laparoscopic Gastric Plication is performed by laparoscopic surgery. The greater curvature of the stomach is separated from the greater omentum using a harmonic scalpel starting approximately 3-5 cm from the pylorus and ending at the angle of His. As needed, adhesions to the posterior surface of the stomach may be transected. At least two rows stitches will be placed laparoscopically about the greater curvature of the stomach starting at or near the angle of His and ending in the antrum. A calibration tube or endoscope will be used to maintain a lumen during the procedure, ensuring one exists after the procedure.

Laparoscopy and da Vinci robotic medical negligence claims are increasing in number because the incidence of the procedure becomes more and much more common. The laparoscope or endoscope is an instrument that resembles a thin telescope which has a fibre-optic light system make it possible for a doctor to determine inside the abdomen. The laparoscope is inserted in to the abdomen through a small incision and co2 gas is then introduced to distend the abdominal cavity make it possible for the contents to be seen. The tools are subsequently inserted into secondary incisions and may be guided by the fibre optic camera that is already in position.
